【摘要】：目的探討低溫等離子輔助懸雍垂腭咽成形術(shù)(UPPP)對(duì)體位性和非體位性阻塞性睡眠呼吸暫停低通氣綜合征(OSAHS)患者的治療效果。方法選擇該院2014年1月至2015年12月經(jīng)多導(dǎo)睡眠呼吸監(jiān)測(cè)(PSG)確診為OSAHS并接受低溫等離子輔助UPPP的患者26例,根據(jù)不同睡眠體位狀態(tài)下呼吸暫停低通氣指數(shù)(AHI)將OSAHS患者分為體位性睡眠呼吸暫停(PPs)組、非體位性睡眠呼吸暫停(NPPs)組,比較兩組患者手術(shù)前、后AHI變化及手術(shù)有效率。結(jié)果 PPs組總AHI、仰臥位AHI(S-AHI)、側(cè)臥位AHI(L-AHI)均低于NPPs組(P0.05),且血氧相關(guān)指標(biāo)高于NPPs組(P0.05);OSAHS患者手術(shù)總有效率為73.08%(19/26),其中PPs組手術(shù)有效率為100%(7/7),NPPs組手術(shù)有效率為63.16%(12/19),但兩組患者手術(shù)有效率比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P=0.13)。與手術(shù)前比較,兩組患者手術(shù)后總AHI、S-AHI、L-AHI均下降(P0.05);NPPs組L-AHI下降幅度明顯高于S-AHI[0.96(0.86,1.00)vs.0.53(0.34,0.77),P0.01]。NPPs組中術(shù)后有78.95%(15/19)轉(zhuǎn)變?yōu)镻Ps。結(jié)論低溫等離子輔助UPPP治療對(duì)OSAHS患者有一定治療效果,其中NPPs患者受益更明顯。
[Abstract]:Objective to investigate the therapeutic effect of hypothermic plasma assisted uvulopalatopharyngoplasty (UPPP) on patients with postural and non-postural obstructive sleep apnea hypopnea syndrome (OSAHS). Methods from January 2014 to December 2015, 26 patients with OSAHS and received hypothermic plasma assisted UPPP were selected. According to apnea hypopnea index (AHII) in different sleep posture, OSAHS patients were divided into two groups: postpositional sleep apnea group and non-postpositional sleep apnea group. The changes of AHI before and after operation and the effective rate were compared between the two groups. Results in the PPs group, the total AHI, the supine AHIS AHIN and the lateral lying AHIG L-AHI) were lower than those in the NPPs group (P 0.05), and the blood oxygen related indexes were higher than that in the NPPs group. The total effective rate was 73.08 鹵19 / 26 in the PPs group, and the effective rate in the PPs group was 63.16% 12 / 19%, but the operative response rate was 63.16% in the PPs group, but the total effective rate was 73.08% / 26% in the NPPs group. There was no significant difference in efficiency. Compared with those before operation, the decrease of L-AHI in the total AHI S-AHIN L-AHI group after operation was significantly higher than that in the S-AHI group [0.96 鹵0.86 鹵1.00 vs.0.530.34 鹵0.77 P0.01]. In the NPPs group, 78.9515% of the patients changed to PPs after operation. Conclusion the hypothermic plasma-assisted UPPP therapy has some therapeutic effect on OSAHS patients, and the benefit of NPPs patients is more obvious.
