【摘要】：目的評估上直肌、外直肌肌腹聯(lián)合術(shù)治療高度近視限制性內(nèi)、下斜視的臨床效果。方法回顧性分析16例27只高度近視限制性斜視眼患者的病例資料,所有患者患眼采用上直肌、外直肌肌腹聯(lián)合術(shù)。隨訪時間3~12月,比較手術(shù)前、后患者的斜視度及眼球運動度。結(jié)果所有患者術(shù)前患眼均有不同程度的上轉(zhuǎn)和外轉(zhuǎn)受限,術(shù)前平均內(nèi)斜視度為(92.50±18.17)△,平均下斜視度為(21.88±5.74)△,眼球外轉(zhuǎn)受限程度為-3.19±0.62,眼球上轉(zhuǎn)受限程度為-2.85±0.82;術(shù)后患者眼球外轉(zhuǎn)(外轉(zhuǎn)受限程度為-1.15±0.52)、上轉(zhuǎn)(上轉(zhuǎn)受限程度為-0.78±0.51)均有明顯提高,與術(shù)前比較,差異有統(tǒng)計學意義(P0.01),內(nèi)斜視度[平均為(9.19±4.39)△]及下斜視度[平均為(2.38±3.10)△]明顯減小,與術(shù)前比較,差異有統(tǒng)計學意義(P0.01),除術(shù)前斜視度較大的個別患者外,總體效果較為滿意(包括獲得較為滿意的外觀)。結(jié)論上直肌、外直肌肌腹聯(lián)合術(shù)能矯正高度近視引起的限制性內(nèi)下斜視,獲得較為滿意的外觀。
[Abstract]:Objective to evaluate the clinical effect of superior rectus muscle and external rectus muscle abdominal surgery in the treatment of high myopia. Methods the data of 27 cases of high myopia with restricted strabismus were analyzed retrospectively. All the patients were treated with superior rectus muscle and external rectus muscle combined with abdominal surgery. The follow-up time from 3 to 12 months was compared with the degree of strabismus and eyeball movement before and after operation. Results all the patients had different degrees of constriction of upturn and extroversion. The mean degree of esotropia was 92.50 鹵18.17) and the mean degree of hypotropia was 21.88 鹵5.74). The degree of exophthalmos limitation was -3.19 鹵0.62, and the degree of constriction was -2.85 鹵0.82.The degree of exophthalmos limitation was -1.15 鹵0.52U, and the degree of up-turn was -0.78 鹵0.51, which was significantly higher than that before operation. The differences were statistically significant (P 0.01), esotropia (9.19 鹵4.39) and hypotropia (2.38 鹵3.10), respectively. The overall effect is satisfactory (including obtaining satisfactory appearance). Conclusion Superior rectus and external rectus combined with abdominal surgery can correct restrictive hypotropia caused by high myopia and obtain satisfactory appearance.
