【摘要】：目的:觀察鼓膜成形術(shù)與藥物保守治療單純慢性化膿性中耳炎的近遠(yuǎn)期療效。方法:將2011年1月至2013年1月收治的80例單純慢性化膿性中耳炎患者作為研究對(duì)象,按完全雙盲法分為對(duì)照組與觀察組2組,每組各40例。對(duì)照組僅給予藥物保守治療,觀察組則在藥物治療的基礎(chǔ)上給予鼓膜成形術(shù)治療,比較2組治療前后聽(tīng)力恢復(fù)情況,統(tǒng)計(jì)2組治療不良反應(yīng)發(fā)生率,2組均隨訪24個(gè)月,統(tǒng)計(jì)患者近遠(yuǎn)期療效及復(fù)發(fā)情況。結(jié)果:觀察組治療6月、12月、24月后氣道骨差分別為(15.2±2.6)d B、(10.6±3.1)d B、(9.1±2.3)d B,與對(duì)照組相比差異有統(tǒng)計(jì)學(xué)意義(P0.05);隨訪6月、12月、24月,觀察組聽(tīng)力改善率(93.2%、95.5%、97.7%)及鼓膜愈合率(93.2%、97.7%、97.7%)與對(duì)照組相比差異均有統(tǒng)計(jì)學(xué)意義(P0.05);觀察組治療不良反應(yīng)發(fā)生率為4.5%,復(fù)發(fā)率為2.3%,對(duì)照組不良反應(yīng)發(fā)生率及復(fù)發(fā)率分別為31.0%、16.7%,2組上述指標(biāo)對(duì)比差異有統(tǒng)計(jì)學(xué)意義(P0.05);觀察組隨訪6個(gè)月治療有效率為93.2%,末次隨訪有效率為97.7%,對(duì)照組則分別為57.1%、78.6%,2組近遠(yuǎn)期療效對(duì)比差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:在單純型慢性化膿性中耳炎患者的臨床治療中,采用保守藥物處理聯(lián)合鼓膜成形術(shù)方案,可促進(jìn)鼓膜愈合,改善聽(tīng)力,降低不良反應(yīng)發(fā)生率,優(yōu)化近遠(yuǎn)期療效,復(fù)發(fā)率低,值得推廣。
[Abstract]:Objective: to observe the short-and long-term effect of tympanoplasty and drug conservative treatment on chronic suppurative otitis media. Methods: 80 cases of simple chronic suppurative otitis media from January 2011 to January 2013 were divided into control group and observation group with 40 cases in each group. The patients in the control group were treated with conservative drug therapy, while the observation group were treated with tympanoplasty on the basis of drug therapy. The hearing recovery of the two groups was compared before and after treatment. The incidence of adverse reactions in the two groups were all followed up for 24 months. The short-and long-term curative effect and recurrence of the patients were analyzed. Results: in the observation group, the difference of airway bone in 6 months, 12 months and 24 months after treatment was 15. 2 鹵2. 6 鹵2. 6 鹵3. 1 鹵3. 1 鹵2. 3 dB, respectively, which was significantly different from that of the control group (P 0. 05), and followed up for 6, 12, 24 months. The rate of hearing improvement and the rate of healing of tympanic membrane in the observation group were significantly higher than those in the control group (P 0.05), the incidence of adverse reactions in the observation group was 4.5, the recurrence rate was 2.3%, and the adverse reaction rate and the recurrence rate in the control group were 31. 0% and 16. 716.716. 716. 7% respectively. The effective rate of treatment was 93.2% in the observation group, 97.7% in the last follow-up group, and 57.1% in the control group (P 0.05). Conclusion: in the clinical treatment of simple chronic suppurative otitis media, conservative drug treatment combined with tympanoplasty can promote the healing of tympanic membrane, improve hearing, reduce the incidence of adverse reactions and optimize the short-term and long-term results. Recurrence rate is low, it is worth popularizing.
