阻塞性睡眠呼吸暫停低通氣綜合征患者骨密度及血清OPG、RANKL的變化

【摘要】:目的通過(guò)測(cè)定阻塞性睡眠呼吸暫停低通氣綜合征(obstructive sleep apnea hypopnea syndrome,OSAHS)患者骨密度及血清骨保護(hù)素(osteoprotegerin,OPG)、核因子κB受體活化因子配體(receptor activator of nuclear factor kappa-B ligand,RANKL)的變化,初步探討OSAHS與骨質(zhì)疏松的關(guān)系。方法根據(jù)呼吸暫停低通氣指數(shù)(apnea hypopnea index,AHI)將我院確診的88名OSAHS患者分為3組:輕度組(5次/h≤AHI15次/h)30例、中度組(15次/h≤AHI30次/h)30例、重度組(AHI≥30次/h)28例,并選擇30名健康人群作為正常對(duì)照組。記錄所有受試者一般資料,測(cè)定骨代謝指標(biāo)及腰椎、股骨頸骨密度,采用雙抗體夾心酶聯(lián)免疫吸附法檢測(cè)各組受試者的血清OPG、RANKL水平,比較各組之間的差異并與AHI進(jìn)行相關(guān)性分析,P0.05為差異有統(tǒng)計(jì)學(xué)意義。結(jié)果中度OSAHS組腰椎、股骨頸骨密度較正常對(duì)照組降低,差異有統(tǒng)計(jì)學(xué)意義;重度OSAHS組腰椎、股骨頸骨密度分別較正常對(duì)照組、輕度OSAHS組降低,差異有統(tǒng)計(jì)學(xué)意義。各組OSAHS患者血清OPG均較正常對(duì)照組降低,其中重度OSAHS組分別與輕度OSAHS組、正常對(duì)照組差異有統(tǒng)計(jì)學(xué)意義。各組OSAHS患者血清RNAKL均較正常對(duì)照組升高,其中中度OSAHS組較正常對(duì)照組差異有統(tǒng)計(jì)學(xué)意義,重度OSAHS組分別較輕度OSAHS組、正常對(duì)照組差異有統(tǒng)計(jì)學(xué)意義。Pearson相關(guān)性分析結(jié)果顯示:骨密度、OPG與AHI呈負(fù)相關(guān),RANKL與AHI呈正相關(guān)。結(jié)論OSAHS患者有發(fā)生骨質(zhì)疏松的風(fēng)險(xiǎn),且與缺氧程度密切相關(guān),OPG/RANKL比率下調(diào)可能為作用機(jī)制之一。
