【摘要】：目的探討抗血管內(nèi)皮生長因子(vascular endothelial growth factor,VEGF)藥物康柏西普對增生型糖尿病視網(wǎng)膜病變(proliferative diabetic retinopathy,PDR)患者玻璃體切割術(shù)(pars plana vitrectomy,PPV)術(shù)中和術(shù)后的影響。方法將2016年1月至12月于我科確診PDR伴有玻璃體出血(vitreous hemorrhage,VH)和(或)牽拉性視網(wǎng)膜脫離(tractional retinal detachment,TRD)的123例149眼患者納入研究,其中康柏西普組64例78眼,對照組59例71眼。康柏西普組于PPV術(shù)前3 d玻璃體內(nèi)注射抗VEGF藥物0.50 mg(0.05 mL),對照組無處理。無明顯并發(fā)癥后2組行標準23G微創(chuàng)鞏膜三通道玻璃體切割術(shù),記錄并分析手術(shù)時間、術(shù)中出血、醫(yī)源性視網(wǎng)膜裂孔、眼內(nèi)電凝使用、是否硅油填充等,對術(shù)前及術(shù)后1個月視力及黃斑厚度進行對比,觀察術(shù)后并發(fā)癥情況。結(jié)果兩組患者年齡、性別、糖化血紅蛋白、糖尿病病程,VH≥Ⅲ級眼數(shù)(56/78、45/71),術(shù)前TRD比較均無明顯差異。康柏西普預處理可明顯降低PPV術(shù)中出血(43/78、49/71)、醫(yī)源性視網(wǎng)膜裂孔的概率(11/78、21/71),減少眼內(nèi)電凝使用(57/78、62/71)及硅油填充(43/78、51/71),進而縮短手術(shù)時間(58.63±21.66)s、(72.69±22.48)s,且其可明顯改善術(shù)后視力(0.23±0.15,0.16±0.11)及黃斑水腫厚度(260.95±27.44)μm、(330.81±36.62)μm,同時降低二次積血發(fā)生率(3/78、10/71)。結(jié)論康柏西普預處理聯(lián)合PPV是治療PDR患者一項積極有效的治療方案。
[Abstract]:Objective to investigate the effect of anticancer vascular endothelial growth factor (VEGF) on proliferative diabetic retinopathy (PDR) in patients with proliferative diabetic retinopathy (PDR) during and after vitrectomy. Methods from January to December 2016, 123 patients (149 eyes) with PDR with vitreous hemorrhage and / or traction retinal detachment diagnosed in our department were included in the study, including 64 cases (78 eyes) and 59 cases (71 eyes) of control group. Compactopril group was injected intravitreous with anti VEGF drug 0. 50 mg(0.05 mL 3 days before PPV, while the control group was not treated. After no obvious complications, the two groups were treated with standard 23G minimally invasive three-channel scleral vitrectomy.
