【摘要】：目的探索以組織嗜酸粒細(xì)胞(Eos)浸潤程度來分類伴鼻息肉型慢性鼻-鼻竇炎(CRSw NP)可能存在的臨界值。方法收集140例CRSw NP患者術(shù)前臨床資料,術(shù)中收集鼻息肉組織評(píng)估組織Eos,術(shù)后3個(gè)月和6個(gè)月時(shí)隨訪。結(jié)果將組織Eos以≤50個(gè)Eos/高倍鏡視野(HPF)為臨界點(diǎn)分組時(shí),術(shù)前視覺模擬、鼻竇CT、鼻內(nèi)鏡評(píng)分,術(shù)后3個(gè)月及6個(gè)月鼻內(nèi)鏡評(píng)分差異有統(tǒng)計(jì)學(xué)意義(P0.05);變應(yīng)原皮膚試驗(yàn)陽性組與陰性組的組織Eos間差異無統(tǒng)計(jì)學(xué)意義;組織Eos與外周血Eos百分比及絕對(duì)值在置信度為0.01時(shí)呈顯著相關(guān)性(ρ=0.459、0.473,P0.01)。結(jié)論對(duì)區(qū)分嗜酸粒細(xì)胞型鼻竇炎與非嗜酸粒細(xì)胞型鼻竇炎最有意義的臨界點(diǎn)可能在50個(gè)Eos/HPF附近(取息肉組織,隨機(jī)HPF);變應(yīng)性因素可能對(duì)CRSw NP患者鼻息肉組織的Eos增多無影響或影響不大;外周血Eos可能能夠成為代表組織Eos高低的一項(xiàng)指標(biāo)。
[Abstract]:Objective to explore the possible critical value of tissue eosinophil (Eos) infiltration to classify chronic rhinosinusitis (CRSw NP) with nasal polyps. Methods 140 patients with CRSw NP were collected before operation, and the nasal polyp tissue was collected to evaluate the tissue Eos, 3 months and 6 months after operation. The result was to organize Eos with less than 50 Eos/ high magnification lenses. When the visual field (HPF) was divided into critical points, preoperative visual simulation, sinus CT, nasal endoscopy score, 3 months and 6 months after the operation were statistically significant (P0.05), and there was no statistical difference between the positive group and the negative group Eos; the percentage and absolute value of Eos in the tissue Eos and the peripheral blood were 0.01 when the confidence was 0.01. Significant correlation (rho =0.459,0.473, P0.01). Conclusion the most significant critical point for differentiating eosinophil type sinusitis and non eosinophilic sinusitis may be near 50 Eos/HPF (polyp tissue, random HPF); allergic factors may not affect or have little influence on Eos increase in nasal polyps of CRSw NP patients; peripheral blood Eos may be not affected. It can be an indicator of the level of the organization's Eos.
