【摘要】：目的隨訪青年近視患者LASIK術(shù)后遠(yuǎn)期干眼。方法以LASIK手術(shù)患者為治療組(A組),非手術(shù)單純近視患者為對(duì)照組(B組),分別于術(shù)前及術(shù)后1 a、3 a、5 a檢查患者裸眼視力、屈光度、淚膜破裂時(shí)間、角膜熒光素染色和淚液分泌試驗(yàn),診斷并填寫(xiě)眼表疾病指數(shù)(ocular surface disease index,OSDI)調(diào)查問(wèn)卷。結(jié)果A組術(shù)后干眼發(fā)病率高于術(shù)前(35.6%)且隨時(shí)間降低(術(shù)后1 a、3 a、5 a分別為51.7%、44.9%和40.7%),B組各時(shí)間點(diǎn)間差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);兩組比較,術(shù)前差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),術(shù)后其他時(shí)間A組均高于B組(均為P0.05)。A組術(shù)后輕度干眼占多數(shù)但逐年減少(分別為65.6%、52.8%和41.7%),中度和重度干眼構(gòu)成逐年提高,且重度干眼更為明顯(分別為6.6%、11.3%和16.7%;P0.05)。術(shù)后5 a,干眼患者裸眼視力和視覺(jué)功能OSDI評(píng)分均明顯低于非干眼患者(均為P0.05),殘留屈光度和眼部癥狀OSDI評(píng)分明顯高于非干眼患者(均為P0.05)。結(jié)論青年近視患者LASIK術(shù)后干眼長(zhǎng)期存在甚至可能加重,要長(zhǎng)期關(guān)注青年近視患者LASIK術(shù)后干眼進(jìn)展。
[Abstract]:Objective to follow up the long-term dry eyes of young myopia patients after LASIK. Methods the patients with LASIK operation were used as the treatment group and the patients with non-surgical myopia as the control group. The naked eyesight, diopter and lacrimal film rupture time were examined before and after operation for 1 a, 3 a and 5 years, respectively. Corneal fluorescein staining and tear secretion test were used to diagnose and fill out the ocular surface disease index (OSDI) questionnaire. Results the incidence of dry eye in group A was higher than that in group A (35. 6% before operation) and decreased with time (51.7% 44.9% and 40.7% respectively) and there was no significant difference between two groups at different time points (P 0.05). There was no significant difference before operation (P 0.05), but the other time after operation in group A was higher than that in group B (both P0.05A group accounted for the majority but decreased year by year (65.6% and 41.7%, respectively). The composition of moderate and severe dry eyes increased year by year. The severe dry eyes were more obvious (6.6% and 16.7%, P 0.05%, respectively). 5 years after operation, the OSDI scores of naked eye and visual function in dry eye patients were significantly lower than those in non-dry eyes patients (P 0.05). The OSDI scores of residual diopter and ocular symptoms were significantly higher in dry eye patients than those in non-dry eyes patients (P 0.05). Conclusion the dry eye of young myopia patients may be aggravated after LASIK. It is necessary to pay attention to the progress of dry eye in young myopia patients after LASIK for a long time.
