S100A4、MMP-2和MMP-9在鼻咽癌中的表達(dá)及其臨床病理學(xué)意義

【摘要】:目的:研究S100A4、MMP-2和MMP-9在鼻咽癌組織中的表達(dá)及其與鼻咽癌臨床病理參數(shù)的關(guān)系及意義。 方法:采用免疫組化S-P法檢測臨床病理完整的72例鼻咽癌中S100A4、MMP-2和MMP-9蛋白的表達(dá),并采用χ2檢驗(yàn)分析S100A4、MMP-2和MMP-9與臨床病理因素的關(guān)系采用Kaplan-Meier法進(jìn)行生存分析,log-rank法檢驗(yàn)差異性,應(yīng)用spearman等級分析三者之間的關(guān)系。 結(jié)果:(1)S100A4在鼻咽癌組織中的陽性表達(dá)率(76.4%)明顯高于鼻咽慢性炎(15%),差異具有統(tǒng)計(jì)學(xué)意義(P0.05);MMP-2在鼻咽癌組織中的陽性表達(dá)率(69.4%)明顯高于鼻咽慢性炎(0%),差異有統(tǒng)計(jì)學(xué)意義(P0.05);MMP-9在鼻咽癌組織中的陽性表達(dá)率(77.8%)明顯高于鼻咽慢性炎(0%),差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。(2)72例鼻咽癌中,S100A4在淋巴結(jié)轉(zhuǎn)移中,N2~3期中S100A4蛋白的表達(dá)明顯高于N0~1期(P<0.05);在臨床分期中,Ⅲ~Ⅳ期表達(dá)水平明顯高于Ⅰ~Ⅱ期,差異有顯著性;有顱底侵犯的表達(dá)水平明顯高于無顱底侵犯(P<0.05);MMP-2蛋白在淋巴結(jié)分期中,N2~3期中的表達(dá)明顯高于N0~1(P<0.05);臨床分期中,Ⅲ~Ⅳ期明顯高于Ⅰ~Ⅱ期,差異有顯著性(P<0.05);MMP-9蛋白在淋巴結(jié)分期中,N2~3期中的表達(dá)明顯高于N0~1(P<0.05);在臨床分期中,Ⅲ~Ⅳ期明顯高于Ⅰ~Ⅱ期,差異有顯著性(P<0.05)。(3)S100A4陽性組中位生存時間為47月(95%CI:43.8,50.2),5年生存率為36.4%,而S100A4陰性組中位生存時間為58月(95%CI:55.1,60.9),5年生存率為52.9%(p<0.05);MMP-2陽性組中位生存時間為47月(95%CI:44.7,49.3),5年生存率為34%,而MMP-2陰性組中位生存時間為58月(95%CI:53.7,62.3),5年生存率為54.5%(p<0.05);MMP-9陽性組中位生存時間為48月(95%CI:46.1,49.9),5年生存率為34%;而MMP-9陰性組中位生存時間為58月(95%CI:51.2,64.8),5年生存率為54.5%,,(p<0.05)。(4)S100A4表達(dá)陽性的55例中,MMP-2表達(dá)陽性的有47例,表達(dá)陰性的有8例,兩者呈正相關(guān)性(P=0.000,r=0.625)。S100A4表達(dá)陽性55例中,MMP-9表達(dá)陽性的有49例、表達(dá)陰性的有6例,兩者呈正相關(guān)性(P=0.000,r=0.489)。MMP-2與MMP-9的表達(dá)無相關(guān)性(P0.05) 結(jié)論:(1)S100A4、MMP-2和MMP-9蛋白在鼻咽癌中的表達(dá)可作為鼻咽癌惡性程度的參考指標(biāo);S100A4、MMP-2和MMP-9蛋白異常表達(dá)與鼻咽癌發(fā)生有關(guān);(2)S100A4、MMP-2和MMP-9的過表達(dá)與鼻咽癌的淋巴結(jié)轉(zhuǎn)移及臨床分期相關(guān),S100A4的表達(dá)與鼻咽癌的浸潤生長相關(guān);(3)S100A4、MMP-2和MMP-9陽性表達(dá)均與不良預(yù)后相關(guān);(4)S100A4與MMP-2、MMP-9表達(dá)呈正相關(guān)性,MMP-2與MMP-9的表達(dá)無相關(guān)性。
[Abstract]:Objective: to study the expression of S100A4 mMMP-2 and MMP-9 in nasopharyngeal carcinoma (NPC) and its relationship with clinicopathological parameters. Methods: immunohistochemical S-P method was used to detect the expression of S100A4mpMMP-2 and MMP-9 protein in 72 cases of nasopharyngeal carcinoma with clinicopathologic integrity. The relationship between S100A4mpMMP-2 and MMP-9 and clinicopathological factors was analyzed by Kaplan-Meier method. The relationship between the three is analyzed by using spearman grade. Results the positive expression rate of S100A4 in nasopharyngeal carcinoma (76. 4) was significantly higher than that of chronic nasopharyngitis (P 0. 05). The positive rate of MMP-2 in nasopharyngeal carcinoma was significantly higher than that in nasopharyngeal carcinoma (P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05). The positive expression rate of S100A4 in nasopharyngeal carcinoma (77.8%) was significantly higher than that in chronic nasopharyngitis (P 0.05). The expression of S100A4 protein in lymph node metastasis of 72 cases of nasopharyngeal carcinoma was significantly higher than that of N0 stage 1 (P < 0.05), and the expression of S100A4 was significantly higher than that of N0 stage 1 (P < 0.05), and the expression of S100A4 protein in lymph node metastasis was significantly higher in nasopharyngeal carcinoma (P < 0.05) than that in nasopharyngeal carcinoma (P < 0.05).
