【摘要】：目的研究糖尿病視網(wǎng)膜病變經(jīng)氪激光治療后光凝斑視網(wǎng)膜的微觀形態(tài)學(xué)變化。方法對15例非增殖型糖尿病視網(wǎng)膜病變患者行全視網(wǎng)膜氪激光治療,應(yīng)用SD-OCT技術(shù)觀察激光治療后1小時,1周,1個月時光凝斑在視網(wǎng)膜各層的微觀形態(tài)學(xué)變化。結(jié)果光凝治療后1 d,視網(wǎng)膜內(nèi)層結(jié)構(gòu)形態(tài)學(xué)未發(fā)生變化,顯著的結(jié)構(gòu)變化開始于外叢狀層,經(jīng)過外核層,外界膜,肌樣體區(qū),橢圓體區(qū),光感受器外節(jié),交叉區(qū),最后到達色素上皮層。光凝治療后1周,光凝斑的直徑較前明顯減小,界限不清楚;視網(wǎng)膜的內(nèi)層結(jié)構(gòu)的內(nèi)核層、內(nèi)叢狀層向下突出,而外層結(jié)構(gòu)的外叢狀層則完全呈"V"型結(jié)構(gòu)變化,外核層消失。肌樣體區(qū)、橢圓體區(qū)、交叉區(qū)同色素上皮層相互融合,與周圍未損傷區(qū)域形成斷裂。光凝治療后1個月,兩個相鄰光凝斑之間的外叢狀層形成拱門樣結(jié)構(gòu),肌樣體區(qū),橢圓體區(qū),交叉區(qū)這三層所形成的高密度復(fù)合體的厚度變薄,寬度變窄,呈萎縮狀態(tài),色素上皮層的連續(xù)性完好。結(jié)論視網(wǎng)膜各層在激光治療后所發(fā)生的結(jié)構(gòu)變化,進一步驗證了激光對糖尿病視網(wǎng)膜病變的治療作用。
[Abstract]:Objective to study the micromorphological changes of photocoagulation spot retina in diabetic retinopathy treated with krypton laser. Methods Fifteen patients with non-proliferative diabetic retinopathy were treated with whole retinal krypton laser. The microscopic morphologic changes of coagulation spot in all layers of retina were observed by SD-OCT technique at 1 hour and 1 month after laser treatment. Results at 1 day after photocoagulation, the morphology of the inner layer of the retina did not change, and the significant structural changes began in the outer plexiform layer, passing through the outer nuclear layer, the outer membrane, the myoid region, the ellipsoid area, the outer segment of the photoreceptor, the cross region. Finally, it reaches the pigment epithelium. One week after photocoagulation treatment, the diameter of photocoagulation spot was obviously decreased and the boundary was not clear. The inner plexiform layer of the inner layer of the retina protruded downward, while the outer plexiform layer of the outer layer completely showed a "V" structure change and the outer nuclear layer disappeared. The myoid region, ellipsoid region, cross region and pigment epithelium fuse with each other and break with the surrounding undamaged area. One month after photocoagulation treatment, the outer plexiform layer between two adjacent photocoagulation spots formed arched structure, muscle like region, ellipsoid area and cross zone. The thickness and width of the high-density complex formed by these three layers became thinner, narrower and shrunk. The continuity of the pigment epithelium is intact. Conclusion the changes of the structure of retina after laser therapy further verify the therapeutic effect of laser on diabetic retinopathy.
