【摘要】：目的探討多聯(lián)手術(shù)治療晶狀體懸韌帶松弛繼發(fā)急性閉角型青光眼的臨床療效。方法回顧性病例研究。2012年3月至2014年5月在河北省眼科醫(yī)院白內(nèi)障科就診的晶狀體懸韌帶松弛繼發(fā)急性閉角型青光眼患者23例(23眼),采用微切口囊袋內(nèi)預(yù)劈核超聲乳化聯(lián)合張力環(huán)植入、前部玻璃體切割和房角分離術(shù)進行治療。術(shù)后隨訪觀察最佳矯正視力、眼壓、前房深度以及前囊收縮情況、后囊膜混濁程度、人工晶狀體的居中性。結(jié)果術(shù)后3個月最佳矯正視力0.6~1.0者8眼,0.3~0.6者12眼,0.1~0.3者3眼。術(shù)后3個月最佳矯正視力較術(shù)前增加,差異有統(tǒng)計學(xué)意義(t=36.225,P0.01)。所有患者術(shù)后1 d眼壓均下降至正常,為10.3~16.5(14.2±1.9)mm Hg(1 k Pa=7.5 mm Hg),與術(shù)前相比差異有統(tǒng)計學(xué)意義(t=27.902,P0.01)。術(shù)后隨訪3個月,未見眼壓明顯波動。術(shù)中未出現(xiàn)灌注液迷流綜合征,術(shù)后未發(fā)生惡性青光眼。術(shù)后3個月所有患者人工晶狀體居中性良好,未出現(xiàn)囊袋收縮、后發(fā)性白內(nèi)障等并發(fā)癥。結(jié)論采用多聯(lián)手術(shù)治療晶狀體懸韌帶松弛繼發(fā)急性閉角型青光眼可以取得滿意的療效。
[Abstract]:Objective to investigate the clinical effect of multiple operation in the treatment of acute angle closure glaucoma secondary to lens suspension ligament relaxation.Methods from March 2012 to May 2014, 23 patients (23 eyes) with acute angle-closure glaucoma secondary to lens suspension ligament relaxation were treated in cataract department of Hebei ophthalmology hospital from March 2012 to May 2014.Phacoemulsification combined with tension ring implantation,Anterior vitrectomy and angle separation were performed.The best corrected visual acuity, intraocular pressure, anterior chamber depth, anterior capsule contraction, posterior capsule opacity and intraocular lens neuter were observed.Results in 3 months after operation, the best corrected visual acuity (BCVA) was 0.61.0 in 8 eyes with 0.3 or 0.6 in 12 eyes with 0.1 and 0.3 in 3 eyes.The best corrected visual acuity (BCVA) at 3 months after operation was significantly higher than that before operation (P 0.01).Intraocular pressure (IOP) of all patients decreased to normal on the 1st day after operation, 10.3 鹵1.9)mm Hg(1 k Pa=7.5 Pa=7.5 (10.3 鹵16.5U), and the difference was statistically significant compared with that before operation (P 0.01).After 3 months follow-up, no significant fluctuation of intraocular pressure was observed.There was no perfusion vagus syndrome during operation and no malignant glaucoma after operation.At 3 months after operation, all the patients had good intraocular lens (IOL) with no capsular contraction and cataract complications.Conclusion multiple surgery is effective in the treatment of acute angle-closure glaucoma secondary to lens suspension ligament relaxation.
