糖尿病合并白內(nèi)障患者血清及房水中鉻元素含量測定及分析

【摘要】:目的 本研究就國內(nèi)外關于鉻元素與糖尿病患者老年性白內(nèi)障的發(fā)病及病情進展方面的關系進行含量測定比較及相關性的分析,以期對糖尿病患者老年性白內(nèi)障的發(fā)病機制及病情的控制能有更進一步的了解。 方法 收集80例經(jīng)臨床確診的患有老年性白內(nèi)障,核成熟度為3-4級的患者:其中合并2型糖尿病患者有40例(實驗組),非糖尿病患者40例(對照組),術前抽取空腹血并離心出血清保存,白內(nèi)障超聲乳化術前抽取前房水保存,采用石墨爐原子吸收法測定兩組患者血清及房水中鉻元素的含量進行比較及統(tǒng)計學分析,并對兩組血清及房水中鉻元素含量的相關性進行分析。 結果 所有實驗標本經(jīng)過嚴格的實驗操作后均獲得較為滿意的實驗結果。 1、實驗組血清及房水中鉻含量與年齡比較均無顯著相關性(r=0.009,,p0.05,r=-0.038,p0.05),對照組血清及房水中鉻含量與年齡間也無顯著相關性(r=-0.045,p0.05,r=0.192,p0.05),實驗組糖尿病病程與血清及房水中鉻元素含量均無顯著相關性,(r=0.378,p0.05,r=-0.067,p0.05)。 2、兩組相同年齡段間分別比較,75歲以下實驗組血清及房水中鉻含量比對照組低,(t=-4.914,p0.05,t=-4.145,p0.05),75歲以上實驗組血清及房水中鉻含量比對照組低,(t=-4.233,p0.05,t=-4.679,p0.05)。 3、實驗組房水中鉻元素含量為(1.11±0.46)μg/L,顯著低于對照組(2.10±0.89)μg/L,且兩組間比較差異有統(tǒng)計學意義(t=-6.206,p0.05)。 4、實驗組血清中鉻元素含量(3.35±0.92)μg/L,也顯著低于對照組(4.59±0.82)μg/L,兩組間比較差異有統(tǒng)計學意義(t=-6.378,p0.05)。 5、實驗組的血清及房水中鉻元素含量進行相關性分析,結果示兩者間有顯著相關性(r=0.777,p0.05),對照組血清及房水中鉻元素含量進行相關性分析也具有顯著相關性(r=0.608,p0.05)。 結論 1、糖尿病合并白內(nèi)障患者的血清及房水中鉻元素的含量較非糖尿病老年性白內(nèi)障組血清及房水中鉻元素含量偏低。 2、鉻對糖尿病患者血糖及前房中葡萄糖水平都有一定程度的調節(jié)作用,推測其對糖尿病患者白內(nèi)障進展可能有一定控制作用 3、血清中鉻元素含量與房水中鉻元素含量比較具有相關性,故推測通過測定血清中鉻可以在一定程度上評估房水鉻含量。
[Abstract]:PurposeIn this study, the relationship between chromium and the incidence and progression of senile cataract in diabetic patients was analyzed.The aim is to further understand the pathogenesis and control of senile cataract in diabetic patients.MethodEighty cases of senile cataract diagnosed by clinical diagnosis were collected.There were 40 patients with type 2 diabetes mellitus (experimental group) and 40 patients with non-diabetes mellitus (control group). Fasting blood was taken and serum was centrifuged before cataract phacoemulsification, and aqueous humor was extracted before phacoemulsification.The content of chromium in serum and aqueous humor of the two groups was determined by graphite furnace atomic absorption spectrometry. The correlation of chromium content in serum and aqueous humor between the two groups was analyzed.ResultAll the specimens obtained satisfactory experimental results after strict operation.1. There was no significant correlation between chromium content in serum and aqueous humor of experimental group and age. There was no significant correlation between chromium content in serum and aqueous humor of control group and age. There was no significant correlation between chromium content in serum and aqueous humor of control group and age. The course of diabetes in experimental group and the content of chromium in serum and aqueous humor in experimental group were not significantly correlated with the content of chromium in serum and aqueous humor. The course of diabetes in experimental group was related to the content of chromium in serum and aqueous humor.There was no significant correlation between the two groups.2. The chromium content in serum and aqueous humor of the experimental group under 75 years old was lower than that of the control group. The content of chromium in the serum and aqueous humor of the experimental group over 75 years old was lower than that of the control group, and the content of chromium in the serum and aqueous humor of the experimental group was lower than that of the control group.3. The content of chromium in aqueous humor in the experimental group was 1.11 鹵0.46 渭 g / L, which was significantly lower than that in the control group (2.10 鹵0.89) 渭 g / L, and the difference between the two groups was statistically significant.4. The content of Cr in the serum of the experimental group was 3.35 鹵0.92 渭 g / L, which was significantly lower than that of the control group (4.59 鹵0.82) 渭 g / L, and the difference between the two groups was statistically significant.5. The content of chromium in serum and aqueous humor of the experimental group was analyzed. The results showed that there was a significant correlation between the content of Cr in serum and aqueous humor, and the content of Cr in serum and aqueous humor of the control group was also significantly correlated with the content of Cr in the serum and aqueous humor of the control group.Conclusion1. The content of Cr in serum and aqueous humor of diabetic patients with cataract was lower than that in non-diabetic senile cataract group.2. Chromium can regulate blood glucose and glucose level in anterior chamber of diabetic patients to a certain extent. It is speculated that chromium can control the development of cataract in diabetic patients to some extent.3. The content of chromium in serum is correlated with the content of chromium in aqueous humor, so it is speculated that the content of chromium in aqueous humor can be evaluated to a certain extent by determining the content of chromium in serum.
