眼內(nèi)壓連續(xù)測量方法研究

【摘要】:作為僅次于白內(nèi)障的第二大致盲眼病,青光眼嚴(yán)重威脅著人眼的視力。青光眼的主要特征是眼內(nèi)壓力間斷或持續(xù)性升高,其水平超過眼球所能耐受的程度,導(dǎo)致不可逆性視神經(jīng)萎縮、視野縮小。特別對眼壓白天正常而夜間偏高的患者,在眼科門診使用現(xiàn)有的單次眼壓測量儀器難以被發(fā)現(xiàn),從而延誤病情,帶來終身的痛苦。針對眼壓的24小時連續(xù)測量,本文開展了基于兩磁鐵排斥力和壓眼瞼的兩種眼壓測量方法的研究,實現(xiàn)了在眼睛閉合狀態(tài)下,每隔一定時間間隔測量一次眼壓,繪制出24小時眼壓波動圖。 為了系統(tǒng)開展實驗研究,首先建立了尸體兔眼前房加壓灌注高眼壓模型,該模型穩(wěn)定可靠、反應(yīng)靈敏、無干擾,滿足了實驗研究的要求。其次,從兩磁鐵相互作用原理和生物體組織的彈性測量原理出發(fā),闡述了兩種眼壓測量方法的工作原理和測量過程,,通過有限元仿真計算和實驗,證明了該兩種測量方法的可行性和有效性。實驗研究表明:基于兩磁鐵排斥力的眼壓測量方法得到的排斥力與位移曲線上存在波動點,該點與眼內(nèi)壓具有某種對應(yīng)關(guān)系,能夠獲得眼內(nèi)壓大小信息;壓眼瞼式測量方法得到的壓力與位移曲線中某一曲線段的斜率與眼內(nèi)壓具有較好的對應(yīng)關(guān)系,可以用來評估眼內(nèi)壓大小。最終設(shè)計出磁鐵排斥式和壓眼瞼式眼壓測量儀器初步結(jié)構(gòu)。
[Abstract]:As the second leading cause of blindness after cataract, glaucoma is a serious threat to human visual acuity. Glaucoma is characterized by intermittent or sustained increase in intraocular pressure, which exceeds the degree to which the eyeball can tolerate. This results in irreversible atrophy of the optic nerve and narrowing of the visual field. Especially for patients with normal intraocular pressure during the day and high night pressure, it is difficult to detect the use of the existing single IOP measurement instrument in the ophthalmology clinic, thus delaying the condition of the patient. For 24 hours of continuous measurement of intraocular pressure, two methods of intraocular pressure measurement based on repulsive force of two magnets and eyelid pressure are studied in this paper. Intraocular pressure was measured at regular intervals, and 24 hours IOP fluctuation was plotted. In order to carry out the experimental study systematically, a high intraocular pressure model of the anterior chamber of the cadaveric rabbit was established. The model was stable, reliable, sensitive and non-interference, and satisfied the requirements of the experimental study. Based on the principle of interaction between two magnets and the principle of elastic measurement of biological tissue, the working principle and measuring process of two IOP measuring methods are expounded. The finite element simulation calculation and experiment are carried out. The feasibility and effectiveness of the two methods are proved. The experimental results show that there are fluctuation points on the curve of repulsion force and displacement based on the repulsion force of two magnets, which has a certain relationship with the intraocular pressure. The information of intraocular pressure can be obtained, and the slope of a curve in the curve of pressure and displacement obtained by eyelid pressure measurement has a good correspondence with intraocular pressure. It can be used to evaluate the intraocular pressure. Finally, the primary structure of the magnet repellent and eyelid pressure measuring instruments is designed.
