【摘要】：目的:探討在內(nèi)淋巴囊減壓的基礎(chǔ)上同時(shí)行外、后半規(guī)管阻塞治療頑固性梅尼埃病的外科技術(shù)。方法:選擇33例中晚期梅尼埃病患者,均在全身麻醉下完成內(nèi)淋巴減壓,同時(shí)磨開外、后半規(guī)管骨質(zhì),以軟組織填塞外后半規(guī)管管腔。結(jié)果:所有患者均順利完成手術(shù),術(shù)后無面癱、全聾及新生耳鳴發(fā)生;所有患者均有頭朦朧感、不平衡感,其中術(shù)后1周內(nèi)6例語言頻率骨導(dǎo)聽力下降,1例術(shù)后2個月發(fā)生突發(fā)性聽力下降伴旋轉(zhuǎn)性眩暈直至全聾。結(jié)論:該研究設(shè)計(jì)的理論基于減輕內(nèi)淋巴積水壓力與對傳導(dǎo)通路的控制,在切除內(nèi)淋巴囊表面骨質(zhì)的基礎(chǔ)上同時(shí)行外、后半規(guī)管阻塞,外科技術(shù)較易掌握。
[Abstract]:Objective: to explore the surgical technique of treating refractory Meniere's disease by external posterior semicircular canal obstruction on the basis of decompression of endolymphatic sac. Methods: 33 patients with middle and advanced Meniere's disease were treated with endolymphatic decompression under general anesthesia. At the same time, the bone of the posterior semicircular canal and the posterior semicircular canal were filled with soft tissue. Results: all the patients successfully completed the operation without facial palsy, total deafness and neonate tinnitus. All the patients had a hazy head and a sense of imbalance. Within 1 week after operation, 6 cases of speech frequency osteoconductive hearing loss occurred in 1 case with sudden hearing loss with rotational vertigo and total deafness 2 months after operation. Conclusion: the theory of this study is based on reducing the pressure of endolymphatic hydrops. The control of the conduction pathway, On the basis of excision of endolymphatic sac surface bone, the posterior semicircular canal obstruction is easy to master.
