【摘要】：背景和目的測(cè)量季節(jié)性或常年性過敏性結(jié)膜炎患者的淚膜脂質(zhì)層厚度,并將測(cè)定結(jié)果及其他淚液相關(guān)檢查結(jié)果與正常人群進(jìn)行對(duì)比。探索過敏性結(jié)膜炎患者和淚膜脂質(zhì)層厚度變化的原因及同淚膜穩(wěn)定性改變的關(guān)系,以期指導(dǎo)臨床診療。材料和方法選取2016年12月至2017年2月于河南省人民醫(yī)院眼科門診就診的患者,通過詢問病史和癥狀體征診斷為季節(jié)性或常年性過敏性結(jié)膜炎且結(jié)膜刮片細(xì)胞學(xué)檢查發(fā)現(xiàn)存在嗜酸性粒細(xì)胞、嗜酸顆粒和/或肥大細(xì)胞患者52例(104眼)為實(shí)驗(yàn)觀察組,招募無(wú)全身疾患?無(wú)干眼癥和其他眼表疾患51例健康志愿者(102眼)為對(duì)照組,分別進(jìn)行淚膜脂質(zhì)層厚度測(cè)量、淚膜脂質(zhì)層形態(tài)分級(jí)評(píng)分、淚液分泌實(shí)驗(yàn)檢查、淚膜破裂時(shí)間測(cè)定、淚河高度測(cè)量和瞼板腺功能評(píng)估診斷瞼板腺功能障礙。采用SPSS 21.0對(duì)數(shù)據(jù)進(jìn)行統(tǒng)計(jì)分析,比較過敏性結(jié)膜炎患者同健康受試者不同參數(shù)結(jié)果之間的差異性。結(jié)果過敏性結(jié)膜炎患者LLT為56.97±20.43nm,其中淚膜脂質(zhì)層厚度LLT≤60nm為70只眼(約占總數(shù)66.04%),淚膜脂質(zhì)層厚度LLT≥75nm為7只眼(約占總數(shù)6.61%)。健康受試者LLT為87.40±28.17nm,淚膜脂質(zhì)層厚度LLT≤60nm為18只眼(約占總數(shù)17.65%),淚膜脂質(zhì)層厚度LLT≥75nm為64只眼(約占總數(shù)62.75%)。過敏性結(jié)膜炎患者和健康志愿者的淚膜脂質(zhì)層厚度(P0.05)和淚膜脂質(zhì)層檢查陽(yáng)性結(jié)果(LLT≤60nm)眼數(shù)占總眼數(shù)的比率(P0.05)均存在顯著差異性。過敏性結(jié)膜炎患者的淚膜破裂時(shí)間為2.77±1.32s,健康志愿者的淚膜破裂時(shí)間為3.55±1.11s,兩個(gè)組之間存在顯著差異性(P0.05);淚過敏性結(jié)膜炎患者的淚膜脂質(zhì)層形態(tài)分級(jí)評(píng)分為2.91±0.61,健康志愿者的膜脂質(zhì)層形態(tài)分級(jí)評(píng)分為3.08±0.65,兩組之間存在顯著差異性(P0.05)。過敏性結(jié)膜炎患者出現(xiàn)瞼板腺功能障礙眼數(shù)/總眼數(shù)比率為34/40(85%),健康志愿者出現(xiàn)瞼板腺功能障礙的比率為7/32(21.9%),兩組之間存在顯著差異性(P0.05)。結(jié)論過敏性結(jié)膜炎患者的淚膜脂質(zhì)層厚度低于健康受試者,是導(dǎo)致過敏性結(jié)膜炎患者淚膜穩(wěn)定性下降的一個(gè)重要原因。過敏性結(jié)膜炎引起的瞼板腺功能障礙為淚膜脂質(zhì)層厚度變薄的一個(gè)重要原因。
[Abstract]:Background and objective to measure the thickness of tear film lipid layer in patients with seasonal or perennial allergic conjunctivitis. The results of measurement and other tear related examinations were compared with those of normal subjects to explore the causes of changes in the thickness of the lipid layer of the lacrimal film in patients with allergic conjunctivitis and the relationship between the changes in the stability of the lacrimal film and the changes in the thickness of the lipid layer. Materials and methods selected patients from December 2016 to February 2017 in the ophthalmological clinic of Henan Provincial people's Hospital. Eosinophilic granulocytes were found in the patients with seasonal or perennial allergic conjunctivitis and conjunctival scraper cytology, with 52 patients with eosinophil and / or mast cells (104 eyes) as experimental observation group.
