【摘要】：目的:觀察丹參酮聯(lián)合角膜緣干細(xì)胞移植術(shù)治療翼狀胬肉的臨床療效和安全性。方法:采用回顧性分析方法,選取我院2010年2月-2014年9月收治的原發(fā)性翼狀胬肉患者97例(118眼),根據(jù)用藥情況分為觀察組48例(57眼)和對照組49例(61眼)。兩組患者均行自體角膜緣干細(xì)胞移植術(shù)。對照組患者于術(shù)前1周開始給予妥布霉素地塞米松滴眼液1~2滴,每4~6 h 1次,行常規(guī)治療;觀察組患者在對照組基礎(chǔ)上給予丹參酮膠囊0.5 g,po,tid,連用3個(gè)月。比較兩組患者角膜上皮修復(fù)時(shí)間和局部癥狀消退時(shí)間,觀察兩組患者術(shù)前及術(shù)后1、3個(gè)月角膜散光度和矯正視力情況,并統(tǒng)計(jì)兩組患者復(fù)發(fā)和不良反應(yīng)發(fā)生情況。結(jié)果:觀察組患者角膜上皮修復(fù)時(shí)間、局部癥狀消退時(shí)間均顯著短于對照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。兩組患者術(shù)前及術(shù)后1個(gè)月角膜散光、矯正視力比較,差異無統(tǒng)計(jì)學(xué)意義(P0.05);與術(shù)前比較,術(shù)后1、3個(gè)月兩組患者角膜散光度顯著降低、矯正視力顯著升高,且術(shù)后3個(gè)月觀察組顯著優(yōu)于對照組,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。觀察組患者復(fù)發(fā)率為3.51%,顯著低于對照組的14.75%,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。兩組患者不良反應(yīng)發(fā)生率比較,差異無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:丹參酮聯(lián)合自體角膜緣干細(xì)胞移植術(shù)治療翼狀胬肉,能顯著縮短角膜上皮修復(fù)和局部癥狀消退時(shí)間,恢復(fù)患者視功能,降低復(fù)發(fā)率,且安全性好。
[Abstract]:Objective: to observe the clinical efficacy and safety of tanshinone combined with limbal stem cell transplantation in the treatment of pterygium. 97 cases of primary pterygium treated in our hospital from February 2010 to September 2014 were divided into observation group (48 cases, 57 eyes) and control group (49 cases, 61 eyes). The patients in the control group were given tobramycin dexamethasone eye drops 1 to 2 drops one week before operation. The patients in the observation group were treated with tanshinone capsule 0.5 g potid for 3 months on the basis of the control group. The corneal epithelium repair time and the time of local symptom regression were compared between the two groups. The corneal astigmatism and corrected visual acuity were observed before and 1, 3 months after operation. The regression time of local symptoms was significantly shorter than that of the control group (P 0.05). There was no significant difference in corrected visual acuity between the two groups in corneal astigmatism before and 1 month after operation (P 0.05), and there was no significant difference between the two groups before and after operation. At 1 and 3 months after operation, the corneal astigmatism and corrected visual acuity of the two groups were significantly decreased, and the observation group was significantly better than the control group 3 months after operation. The recurrence rate of the patients in the observation group was 3.51, which was significantly lower than that in the control group (14.75%), and the difference was statistically significant (P 0.05). The incidence of adverse reactions in the two groups was higher than that in the control group. Conclusion: the treatment of pterygium with tanshinone combined with autogenous limbal stem cell transplantation can significantly shorten the time of corneal epithelial repair and local symptom regression, restore the visual function of the patients, and reduce the recurrence rate, and the safety is good.
