【摘要】：目的:了解我國(guó)耳鼻咽喉疾病的臨床誤診概況,為減少誤診、提高診斷水平提供參考依據(jù)。方法:利用誤診疾病數(shù)據(jù)庫(kù)檢索與管理系統(tǒng)對(duì)數(shù)據(jù)庫(kù)已收錄的2004-2013年誤診文獻(xiàn)數(shù)據(jù)中所有耳鼻咽喉疾病文獻(xiàn)進(jìn)行統(tǒng)計(jì)學(xué)分析,包括文獻(xiàn)來(lái)源、樣本量、誤診率、誤診范圍、誤診后果、誤診原因等。結(jié)果:共命中文獻(xiàn)369篇,累計(jì)誤診病例4 211例。共51種確診疾病,平均誤診率為25.43%,誤診率最高為鼻咽結(jié)核(84.76%)、結(jié)核性中耳炎(75.00%)、先天性喉囊腫(75.00%),最低為鼻竇炎(5.92%)。誤診病例數(shù)最高的前3位疾病依次為鼻咽喉結(jié)核(1 216例)、鼻-鼻竇炎(710例)和良性陣發(fā)性位置性眩暈(697例)。對(duì)誤診后果的統(tǒng)計(jì)顯示,97.22%的患者為Ⅲ級(jí)誤診后果,即發(fā)生誤診誤治未造成不良后果,但仍有10例造成Ⅰ級(jí)后果(死亡或后遺癥)。對(duì)文獻(xiàn)中誤診原因頻次進(jìn)行統(tǒng)計(jì),居前3位的依次為經(jīng)驗(yàn)不足缺乏對(duì)該病的認(rèn)識(shí),問(wèn)診及體檢不細(xì)致和未選擇針對(duì)性檢查措施。結(jié)論:10年誤診大數(shù)據(jù)結(jié)果在一定程度上反映了我國(guó)耳鼻咽喉疾病的誤診現(xiàn)象,耳鼻咽喉科和神經(jīng)科、口腔科、呼吸科及眼科醫(yī)生都應(yīng)了解容易誤診的耳鼻咽喉疾病的鑒別診斷要點(diǎn),力求減少臨床誤診誤治。
[Abstract]:Objective: to understand the general situation of clinical misdiagnosis of otorhinolaryngology in China, in order to reduce misdiagnosis. Methods: by using the database retrieval and management system of misdiagnosis diseases, all the literature on otorhinolaryngopharynx diseases in the misdiagnosis literature data collected from 2004 to 2013 in the database were analyzed statistically, including the sources of the literatures. Results: a total of 369 documents were hit, 4 211 cases were misdiagnosed, 51 kinds of diseases were diagnosed. The average misdiagnosis rate was 25.43. The highest misdiagnosis rate was nasopharyngeal tuberculosis (84.76C), tuberculous otitis media (75.00), congenital laryngeal cyst (75.00), and nasal sinusitis (5.922um). The top three diseases with the highest number of misdiagnosed cases were nasopharyngeal tuberculosis (1 216 cases), rhinosinosinusitis (710). And 697 cases of benign paroxysmal positional vertigo.
