【摘要】：目的總結(jié)喉聲門上水平部分切除術(shù)治療喉癌的療效和經(jīng)驗(yàn)。方法收集本院2002—2009年99例接受喉聲門上水平部分切除術(shù)患者的臨床資料。利用UICC2002標(biāo)準(zhǔn)分期,選擇相應(yīng)的淋巴結(jié)清掃術(shù)。對(duì)患者的5年生存率、術(shù)后并發(fā)癥和喉功能進(jìn)行評(píng)估。結(jié)果 99例患者中,有94例完成了5年隨訪,隨訪率為94.9%;死亡31例,5年總生存率為68.7%。經(jīng)頸淋巴結(jié)病理檢查示,無(wú)轉(zhuǎn)移、有轉(zhuǎn)移患者的術(shù)后5年生存率分別為76.2%、48.4%,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P0.05)。拔管率為88.9%,患者吞咽功能基本恢復(fù),發(fā)音功能良好。主要并發(fā)癥為局部感染,發(fā)生率為10.1%,抗感染均治愈。隨診發(fā)現(xiàn)致死原因主要為頸淋巴轉(zhuǎn)移,14例(45.2%)。結(jié)論喉聲門上水平部分切除術(shù)對(duì)部分聲門上型喉癌具有理想的治療效果,但是對(duì)于頸側(cè)清掃術(shù)似乎需要更理想的選擇標(biāo)準(zhǔn)。
[Abstract]:Objective to summarize the efficacy and experience of supraglottic partial laryngectomy in the treatment of laryngeal cancer. Methods the clinical data of 99 patients undergoing supraglottic partial laryngectomy from 2002 to 2009 were collected. The 5-year survival rate, postoperative complications and laryngeal function were evaluated. The follow-up rate was 94.9 and 31 cases died. The 5-year overall survival rate was 68.7. The pathological examination of cervical lymph nodes showed that there was no metastasis. The 5-year survival rate of the patients with metastasis was 76.2and 48.4, respectively. The difference between the two groups was statistically significant (P 0.05). The extubation rate was 88.9%. The patients' swallowing function was basically recovered and the pronunciation function was good. The main complication was local infection. The incidence rate was 10.1 and the anti-infection was cured. 14 cases of cervical lymphatic metastasis were found to be the main cause of death. Conclusion the supraglottic horizontal partial laryngectomy has an ideal therapeutic effect for partial supraglottic laryngeal carcinoma. But for the side neck dissection, it seems that a better choice is needed.
