【摘要】：目的： 評價有晶體眼后房型環(huán)曲面人工晶狀體植入術(shù)矯正高度近視合并散光的有效性、安全性及穩(wěn)定性。 方法： 觀察2010年3月至2011年12月于浙江大學(xué)醫(yī)學(xué)院附屬第二醫(yī)院眼科中心行有晶體眼后房型環(huán)曲面人工晶狀體(Toric implantable collamer lens, TICL)植入術(shù)的高度近視合并散光患者8例(15只眼),術(shù)前平均近視度數(shù)為(-13.72±2.71)D,平均散光度數(shù)為(-3.15±1.08)D。術(shù)后1天、1周、1月、3月進行隨訪,隨訪內(nèi)容包括裸眼視力、最佳矯正視力、球鏡度數(shù)、散光度數(shù)、裂隙燈顯微鏡檢查、眼底檢查、眼壓、角膜內(nèi)皮細胞計數(shù)、中央角膜厚度、拱高、TICL軸向旋轉(zhuǎn)情況。 結(jié)果： 全部8例(15只眼)TICL植入均一次成功。術(shù)后3月,術(shù)眼球鏡度數(shù)由術(shù)前(-13.72±2.71)D降低至(-0.37±0.88)D(P0.01),散光度數(shù)由(-3.15±1.08)D矯正至(-0.57±0.45)D(P0.01),具有統(tǒng)計學(xué)意義。術(shù)眼裸眼視力由術(shù)前3.48±0.10(5-logMAR)提高到術(shù)后4.84±0.14(5-logMAR),最佳矯正視力由術(shù)前4.84±0.14(5-logMAR)提高到術(shù)后4.95±0.10(5-logMAR)(P0.05),有統(tǒng)計學(xué)意義。術(shù)后3月觀察TICL散光軸向,平均旋轉(zhuǎn)度數(shù)絕對值為(3.4±1.5)°。術(shù)后3個月隨訪眼壓、角膜內(nèi)皮細胞計數(shù)、中央角膜厚度與術(shù)前相比均無明顯變化(P0.05)。1眼(6.7%)術(shù)后1天眼壓一過性升高,2眼(13.3%)術(shù)后發(fā)生夜間眩光,1眼(6.7%)術(shù)后3月隨訪時見自身晶狀體前囊下輕度混濁。所有術(shù)眼隨訪期間均未發(fā)現(xiàn)瞳孔阻滯、色素播散性青光眼、眼內(nèi)炎等術(shù)后并發(fā)癥。 結(jié)論： 有晶體眼后房型環(huán)曲面人工晶狀體植入術(shù)矯正高度近視合并散光在隨訪期內(nèi)具有有效性、安全性及穩(wěn)定性。遠期效果需進一步觀察。
[Abstract]:Objective:. To evaluate the efficacy, safety and stability of posterior chamber curved intraocular lens implantation (IOL) for correction of high myopia with astigmatism. Methods:. To observe 8 patients with high myopia complicated with astigmatism treated with posterior chamber implantable collamer lens implantation from March 2010 to December 2011 in the ophthalmology center of the second affiliated Hospital of Zhejiang University Medical College. The mean preoperative myopia and astigmatism were -13.72 鹵2.71d and -3.15 鹵1.08d respectively. The follow-up included naked visual acuity, best corrected visual acuity, spherical degree, astigmatism, slit lamp microscopy, fundus examination, intraocular pressure, corneal endothelial cell count, central corneal thickness, arch height and axial rotation of TICL. Results:. TICL implantation was successful in all 8 cases with 15 eyes. On March, TICL was implanted successfully in all eyes. The degree of ophthalmoscopy decreased from -13.72 鹵2.71D before operation to -0.37 鹵0.88D, and astigmatism was corrected from -3.15 鹵1.08D to -0.57 鹵0.45g / g P 0.01.The naked visual acuity increased from 3.48 鹵0.105logMARbefore operation to 4.84 鹵0.145logMARL, and the best corrected visual acuity increased from 4.84 鹵0.145logMARbefore operation to 4.95 鹵0.105-MARP0.05L after operation. TICL astigmatism axis was observed on March, The mean degree of rotation was 3.4 鹵1.5 擄. The intraocular pressure (IOP) and corneal endothelial cell count were observed 3 months after operation. There was no significant change in the thickness of central cornea between before and after operation (P 0.05% or 6.7%) IOP was temporarily elevated in 2 eyes (13.3%) 1 day after operation. (1 eye had nocturnal glare and 6.7%) on March, mild opacification under anterior lens capsule was observed. All eyes were treated with subcapsular opacification. No pupillary block was found during follow-up. Pigment disseminated glaucoma, endophthalmitis and other postoperative complications. Conclusion:. The correction of high myopia combined with astigmatism with posterior chamber curved intraocular lens implantation is effective, safe and stable during the follow-up period.
