【摘要】：目的探討提上瞼肌縮短術(shù)治療上瞼下垂手術(shù)失敗后,利用聯(lián)合筋膜鞘懸吊再次手術(shù)矯正的效果。方法 2008年12月-2014年12月6年間,利用該技術(shù)治療提上瞼肌縮短術(shù)后復(fù)發(fā)中重度上瞼下垂225例346只眼,單側(cè)104例,雙側(cè)121例,在局部麻醉下將瞼板縫合懸吊到位于結(jié)膜上穹窿的上直肌與提上瞼肌之間的聯(lián)合筋膜鞘上,可以有效地提高上眼瞼的高度,術(shù)后切口均一期愈合。結(jié)果隨訪1年到3年,其中滿意273只眼,占79%,改善63只眼,占18%,無效10只眼,占3%。對(duì)10只無效眼及63只改善眼做了二次調(diào)整,調(diào)整時(shí)間最短為術(shù)后3周,最長(zhǎng)為術(shù)后6個(gè)月,調(diào)整后除6只眼為改善,其余均達(dá)到滿意標(biāo)準(zhǔn)。雙瞼瞼裂對(duì)稱,重瞼弧度自然,無瞼裂閉合不全加重。結(jié)論應(yīng)用結(jié)膜上穹窿聯(lián)合筋膜鞘懸吊技術(shù)治療提上瞼肌縮短術(shù)后復(fù)發(fā)性中重度上瞼下垂,既能有效地提高上眼瞼高度,又能獲得很好的美容效果。
[Abstract]:Objective to improve the failure on the eyelid muscle shortening treatment of ptosis after operation, the effect of correction using joint fascial sheath suspension surgery again. Methods from December 2008 -2014 year in December 6 years, the recurrence using this technique for the treatment of levator shortening postoperative ptosis in 225 eyes of 346 cases, 104 cases were unilateral, bilateral 121 cases under local anesthesia, the tarsal plate suture suspension in the conjunctiva fornix of superior rectus muscle and upper eyelid muscle joint provided between the fascial sheath, can effectively improve the height of the upper eyelid, incision healed after operation. Results the patients were followed up for 1 to 3 years, among them with 273 eyes, accounting for 79%, improved 63 eyes, accounted for 18%, invalid 10 eyes, accounting for 3%. of 10 eyes and 63 eyes improved invalid made two adjustments, adjusted for the shortest time after 3 weeks, the longest was 6 months after surgery, after adjustment for the rest except 6 eyes improved, achieved satisfactory standard. Double eyelid fissure symmetric double eyelid Conclusion: the application of conjunctival fornix combined with fascial sheath suspension for the treatment of recurrent moderate and severe ptosis after shortening the levator palpebrae superioris can effectively improve the upper eyelid height and achieve good cosmetic results.
