CEA、CA153、TPS、Fer、CYFRA21-1檢測(cè)在乳腺癌術(shù)后轉(zhuǎn)移中的診斷價(jià)值

【摘要】:目的探索腫瘤標(biāo)志物CEA、CA153、TPS、Fer、CYFRA21-1在乳腺癌術(shù)后轉(zhuǎn)移中的診斷價(jià)值。方法選擇2012-2014年在唐山市人民醫(yī)院治療的578例原發(fā)性乳腺癌患者,隨訪3年后根據(jù)有、無(wú)轉(zhuǎn)移分為轉(zhuǎn)移組42例,從未出現(xiàn)轉(zhuǎn)移患者中隨機(jī)抽取42例作為未轉(zhuǎn)移組。采用化學(xué)發(fā)光法檢測(cè)患者CEA、CA153、TPS、Fer、CYFRA21-1水平,比較轉(zhuǎn)移組與未轉(zhuǎn)移組CEA、CA153、TPS、Fer、CYFRA21-1水平差異;采用Pearson相關(guān)性檢驗(yàn)分析在乳腺癌術(shù)后轉(zhuǎn)移患者中TPS、Fer、CYFRA21-1三者的相關(guān)性;采用受試者ROC曲線檢驗(yàn)各組數(shù)值對(duì)乳腺癌術(shù)后轉(zhuǎn)移的檢驗(yàn)效率。結(jié)果乳腺癌術(shù)后轉(zhuǎn)移組患者血清CEA、CA153、TPS、Fer、CYFRA21-1水平[分別為(6.11±1.87)ng/m L、(26.97±9.52)U/m L、(155.95±22.03)U/L、(173.08±19.87)ng/m L、(18.98±7.33)ng/m L]比未轉(zhuǎn)移組[分別為(5.33±1.85)ng/m L、(23.34±8.30)U/m L、(133.22±19.09)U/L、(142.29±22.59)ng/m L、(6.76±1.59)ng/m L]升高,TPS、Fer、CYFRA21-1水平變化更明顯,差異有統(tǒng)計(jì)學(xué)意義(P0.05);乳腺癌術(shù)后轉(zhuǎn)移患者血清TPS、Fer、CYFRA21-1均未見(jiàn)明顯相關(guān)性;CYFRA21-1的ROC曲線下面積(0.903)Fer(0.849)TPS(0.791),在乳腺癌術(shù)后轉(zhuǎn)移的診斷中,CYFRA21-1曲線下面積最大具有較高的診斷價(jià)值。結(jié)論乳腺癌患者術(shù)后血清TPS、Fer、CY-FRA21-1的水平升高與乳腺癌術(shù)后發(fā)生轉(zhuǎn)移密切相關(guān),CYFRA21-1對(duì)于監(jiān)測(cè)乳腺癌術(shù)后轉(zhuǎn)移具有較高的價(jià)值。
[Abstract]:Objective to explore the diagnostic value of tumor marker CEA,CA153,TPS,Fer,CYFRA21-1 in postoperative metastasis of breast cancer. Methods 578 patients with primary breast cancer treated in Tangshan people's Hospital from 2012 to 2014 were divided into metastasis group (n = 42) and non-metastasis group (n = 42). The level of CEA,CA153,TPS,Fer,CYFRA21-1 was measured by chemiluminescence method, and the difference of CEA,CA153,TPS,Fer,CYFRA21-1 level between metastasis group and non-metastasis group was compared. Pearson correlation test was used to analyze the correlation of TPS,Fer,CYFRA21-1 in patients with postoperative metastasis of breast cancer, and ROC curve was used to test the efficiency of each group in detecting postoperative metastasis of breast cancer. Results the serum CEA,CA153,TPS,Fer,CYFRA21-1 levels in patients with postoperative metastasis of breast cancer were (6.11 鹵1.87) ng/m / L, (26.97 鹵9.52) U / L, (155.95 鹵22.03) U / L, respectively. (173.08 鹵19.87) ng/m / L, (18.98 鹵7.33) ng/m / L, (5.33 鹵1.85) ng/m / L, (23.34 鹵8.30) U / L, (133.22 鹵19.09) U / L, (142.29 鹵22.59) ng/m / L, (6.76 鹵1.59) ng/m / L] increased, and the change of TPS,Fer,CYFRA21-1 level was more obvious, the difference was statistically significant (P 0.05). There was no significant correlation between serum TPS,Fer,CYFRA21-1 in patients with postoperative metastasis of breast cancer. The area under ROC curve of CYFRA21-1 (0.903) Fer (0.849) TPS (0.791). In the diagnosis of postoperative metastasis of breast cancer, the area under CYFRA21-1 curve is the most valuable in the diagnosis of postoperative metastasis of breast cancer. Conclusion the increase of serum TPS,Fer,CY-FRA21-1 level in patients with breast cancer is closely related to postoperative metastasis of breast cancer. CYFRA21-1 is of great value in monitoring postoperative metastasis of breast cancer.
