真皮脂肪瓣聯(lián)合十字縫線支撐矯正重度乳頭內(nèi)陷的臨床觀察

【摘要】：目的探討真皮脂肪瓣聯(lián)合十字縫線支撐矯正重度乳頭內(nèi)陷的臨床效果。方法對15例(29側(cè))重度乳頭內(nèi)陷患者,運用乳暈下真皮脂肪瓣交叉支撐乳頭基底、瓣下以縫線做“十”字交叉支撐、同時外接牽引器牽拉懸吊聯(lián)合療法,術(shù)后隨訪6個月至一年,測量乳頭矯正高度,評估矯正后乳頭形態(tài)。結(jié)果所有患者手術(shù)后至拆線期間均未發(fā)生感染或皮膚壞死并發(fā)癥,切口愈合良好。其中14例患者愈后均無乳頭血運障礙或感覺障礙,1例患者拆線時左側(cè)乳頭感覺稍麻木,術(shù)后三個月時恢復(fù)正常,4例有哺乳需求的患者產(chǎn)后順利哺乳。術(shù)后6個月隨訪平均乳頭矯正高度為0.35cm,醫(yī)患雙方對矯正后的乳頭外觀滿意。結(jié)論真皮脂肪瓣聯(lián)合“+”字縫線支撐法矯正重度乳頭內(nèi)陷,該手術(shù)方法的設(shè)計從乳頭內(nèi)陷的發(fā)病機制出發(fā),針對性的運用解決方案,設(shè)計合理,手術(shù)效果良好,術(shù)后復(fù)發(fā)率低,安全且實用,是較為理想的乳頭內(nèi)陷矯正方法。
[Abstract]:Objective to investigate the clinical effect of dermis fat flap combined with cross suture support in the correction of severe nipple invagination. Methods 15 patients (29 sides) with severe nipple invagination were treated with subareola dermis fat flap cross-supporting nipple base, sutures under the flap for "ten" cross support, and external tractor traction and suspension combined therapy. The patients were followed up for 6 months to 1 year. The height of nipple correction was measured and the shape of nipple after correction was evaluated. Results there were no complications of infection or skin necrosis from operation to disconnection, and the incision healed well. Among them, 14 patients had no nipple blood flow disorder or sensory disturbance, 1 patient felt slightly numb at the time of disconnection, returned to normal three months after operation, and 4 patients with lactation need lactation smoothly after delivery. The average nipple correction height was 0.35 cm at 6 months after operation, and both doctors and patients were satisfied with the appearance of the corrected nipples. Conclusion the combination of dermis fat flap and "suture support method is used to correct severe nipple invagination. The design of this surgical method is based on the pathogenesis of nipple invagination, and the targeted application of the solution is reasonable, and the operation effect is good. The postoperative recurrence rate is low, safe and practical, and it is an ideal method for correction of nipple invagination.
