【摘要】：背景和目的:在皮膚組織受到創(chuàng)傷后,由于受到各方面的影響在愈合過程中會出現(xiàn)膠原纖維的過度增生,形成了增生性瘢痕或者瘢痕疙瘩。各種形式的方法已經(jīng)被用于治療增生性瘢痕,但由于有限的有效性和不可忽視的副作用從而限制了它們的合理應(yīng)用。最近,一種新型的非剝脫性的點陣激光技術(shù)也開始被應(yīng)用于增生性瘢痕的治療中。本文的目的是評估非剝脫性點陣激光治療17例增生性瘢痕患者的有效性及安全性。方法:我科門診就診的增生性瘢痕患者17例被納入研究,給予非剝脫性點陣激光治療。告知患者此次治療共6次,每隔4-6周來院治療一次。治療結(jié)束后隨訪半年。每次治療前及治療結(jié)束后一月由兩名醫(yī)師根據(jù)數(shù)碼相機(jī)拍攝照片及患者瘢痕表象對治療效果進(jìn)行評價,治療前和6次治療后由兩名醫(yī)師進(jìn)行VSS評分。療程結(jié)束后一月由患者本人對治療效果的滿意程度進(jìn)行評分。結(jié)果:17例患者都完成6次治療,次數(shù)增加療效也隨之提高,6次治療后的總有效率為58.8%。根據(jù)溫哥華瘢痕評分量表(VSS)6次治療后的總有效率為64.7%�；颊邼M意度自為58.8%。治療后患者感輕微疼痛,治療部位輕度水腫,局部給予冰袋冰敷30分鐘癥狀緩解。治療結(jié)束后沒有患者治療部位出現(xiàn)水皰、結(jié)痂等。隨訪半年后,1例患者出現(xiàn)治療部位色素沉著,未經(jīng)過任何方法進(jìn)行干預(yù),半年后逐漸好轉(zhuǎn)。結(jié)論:非剝脫性點陣激光是一種有效的、安全的治療增生性瘢痕的方法,值得我們進(jìn)一步研究及應(yīng)用臨床。
[Abstract]:Background and objective: hypertrophic scar or keloid will occur in the process of healing due to the excessive proliferation of collagen fibers in the process of healing after trauma of skin tissue. Various forms of methods have been used in the treatment of hypertrophic scars, but their reasonable application is limited by their limited effectiveness and side effects that can not be ignored. Recently, a new type of non-exfoliative lattice laser technique has also been used in the treatment of hypertrophic scars. The purpose of this study was to evaluate the efficacy and safety of non-stripping lattice laser in the treatment of 17 patients with hypertrophic scar. Methods: seventeen patients with hypertrophic scar in our clinic were treated with non-stripping lattice laser. Inform the patient of this treatment 6 times, every 4 times 6 weeks to come to the hospital for treatment. The patients were followed up for half a year after treatment. Before and one month after each treatment, the therapeutic effect was evaluated by two doctors according to the photos taken by digital camera and the scar appearance of the patients, and the VSS scores were evaluated by two doctors before and 6 times after treatment. One month after the end of the course of treatment, the patients' satisfaction with the therapeutic effect was evaluated. Results: all the 17 patients completed 6 times of treatment, and the curative effect was also improved with the increase of frequency. The total effective rate was 58.8% after 6 times of treatment. According to the Vancouver scar scale (VSS), the total effective rate was 64.7%. The patient satisfaction rate was 58.8%. After treatment, the patients felt mild pain, mild edema in the treatment site, and local ice bag ice compress for 30 minutes to relieve the symptoms. There were no blisters and scabs in the treatment site after treatment. After half a year of follow-up, one patient had pigmentation at the treatment site, and gradually improved after half a year without any intervention. Conclusion: non-stripping lattice laser is an effective and safe method for the treatment of hypertrophic scar, which is worthy of further study and clinical application.
