【摘要】：關(guān)節(jié)假體周圍感染是關(guān)節(jié)置換術(shù)后災(zāi)難性的并發(fā)癥。目前對于關(guān)節(jié)假體周圍感染尚缺乏一種可靠、迅捷、高準確率的診斷方法。美國骨肌系統(tǒng)感染協(xié)會(Musculoskeletal Infection Society,MSIS)制定的關(guān)節(jié)假體周圍感染診斷標準獲得國內(nèi)外學(xué)者的普遍認可,但其診斷指標相對復(fù)雜不便于臨床應(yīng)用。臨床上常用血沉、血清CRP、IL-6、降鈣素原等實驗室指標輔助診斷關(guān)節(jié)假體周圍感染,但這些指標的異常并不完全由關(guān)節(jié)假體周圍感染引起,易受其他炎性因素影響。近年來,滑液生物標志物,尤其是滑液α-防御素對關(guān)節(jié)假體周圍感染的診斷價值逐漸受到重視。本文綜合分析目前關(guān)于滑液α-防御素的臨床研究,就其在關(guān)節(jié)假體周圍感染診斷中的研究現(xiàn)狀及臨床價值做一綜述。
[Abstract]:Periprosthetic infection is a catastrophic complications after joint replacement. At present, there is still a lack of a reliable, rapid and accurate method for the diagnosis of periarthrosis infection. The diagnostic standard of periarthrosis infection established by American Association of Bone and muscle system infection (Musculoskeletal Infection Society,MSIS) has been widely recognized by scholars at home and abroad, but its diagnostic index is relatively complex and not convenient for clinical application. The laboratory indexes such as erythrocyte sedimentation rate (erythrocyte sedimentation rate) and serum CRP,IL-6, calcitonin are often used to assist in the diagnosis of peri-articular infection, but the abnormality of these indexes is not completely caused by peri-articular infection and is easily affected by other inflammatory factors. In recent years, synovial biomarkers, especially synovial 偽-defensin, have been paid more and more attention to the diagnostic value of synovial biomarkers, especially synovial 偽-defensin. In this paper, the current clinical research on synovial 偽-defensin is comprehensively analyzed, and the research status and clinical value of 偽-defensin in the diagnosis of periarthrosis infection are reviewed.
