【摘要】：目的探討電磁導(dǎo)航交鎖髓內(nèi)釘遠(yuǎn)端鎖釘技術(shù)治療股骨干骨折的早期臨床效果。方法 2012年7月-2013年10月,收治53例股骨干骨折。其中男40例,女13例;年齡16~52歲,平均38.3歲。致傷原因:交通事故傷28例,高處墜落傷11例,摔傷7例,擠壓傷4例,其他傷3例。開(kāi)放骨折3例,均為GustiloⅠ度損傷;閉合骨折50例。骨折位于股骨上段17例,中段29例,下段7例。骨折按Winquist分型:Ⅰ型7例,Ⅱ型8例,Ⅲ型22例,Ⅳ型16例;按AO分型:32-A型18例,32-B型28例,32-C型7例。受傷至手術(shù)時(shí)間3~11 d,平均5 d。均采用電磁導(dǎo)航下遠(yuǎn)端交鎖髓內(nèi)釘植入治療。結(jié)果 53例交鎖髓內(nèi)釘?shù)倪h(yuǎn)端鎖釘均在電磁導(dǎo)航下完成,遠(yuǎn)端鎖釘一次成功率達(dá)100%;鎖釘時(shí)間5.0~9.5 min,平均7.0 min。術(shù)后患者切口均Ⅰ期愈合,無(wú)皮膚壞死、感染、竇道形成等術(shù)后早期并發(fā)癥發(fā)生。53例均獲隨訪,隨訪時(shí)間5~12個(gè)月,平均9個(gè)月。1例出現(xiàn)臀部疼痛、臀中肌肌力減弱,取出髓內(nèi)釘后癥狀消失。隨訪期間無(wú)斷釘、退釘、感染、再骨折、外展肌無(wú)力、異位骨化等并發(fā)癥發(fā)生。骨折均獲臨床愈合,愈合時(shí)間8~22周,平均14.5周。術(shù)后8個(gè)月隨訪49例患者,Lys holm評(píng)分獲優(yōu)44例,良4例,可1例,優(yōu)良率98%。結(jié)論電磁導(dǎo)航系統(tǒng)定位精度高、鎖釘時(shí)間短、無(wú)輻射、安全可靠,應(yīng)用該系統(tǒng)行遠(yuǎn)端交鎖髓內(nèi)釘植入可取得較好的早期臨床效果。
[Abstract]:Objective to investigate the early clinical effect of electromagnetic navigation interlocking intramedullary nail in the treatment of femoral shaft fracture. Methods from July 2012 to October 2013, 53 cases of femoral shaft fractures were treated. There were 40 males and 13 females, aged 16 to 52, with an average of 38.3 years old.
