高頻超聲檢測軟組織內(nèi)異物的臨床價值分析

【摘要】：目的探討高頻超聲檢測軟組織內(nèi)異物的臨床價值。方法選擇2016年1—6月江蘇大學(xué)附屬上海第八醫(yī)院普外科收治的215例疑有軟組織異物殘留的入院患者,術(shù)前均行高頻超聲檢測,以實時超聲引導(dǎo)下手術(shù)探查結(jié)果為金標(biāo)準(zhǔn),分析高頻超聲檢測軟組織內(nèi)異物的臨床價值。結(jié)果 215例患者手術(shù)共探查307處軟組織,實際有異物數(shù)275個,實際無異物數(shù)32個。術(shù)前高頻超聲檢測262個異物陽性,誤診3個,45個異物陰性,漏診16個,其靈敏度為94.2%,特異度為90.6%,漏診率為5.8%,誤診率為9.4%,正確率為93.8%,陽性似然比為10.046,陰性似然比為0.064,約登指數(shù)為0.848。結(jié)論高頻超聲廉價、安全、高效,其檢測軟組織內(nèi)異物的正確率高,具有重要的臨床價值,可指導(dǎo)手術(shù)實施。
[Abstract]:Objective to evaluate the clinical value of high-frequency ultrasound in detecting foreign bodies in soft tissue. Methods 215 patients with suspected residual soft tissue foreign bodies were selected from January to June 2016 in the General surgery Department of Shanghai eighth Hospital affiliated to Jiangsu University. All patients were examined by high frequency ultrasound before operation. The results of real-time ultrasound guided exploration were used as the gold standard. To analyze the clinical value of high frequency ultrasound in detecting foreign body in soft tissue. Results 307 soft tissues were explored in 215 cases, 275 foreign bodies and 32 foreign bodies were found. Of 262 foreign bodies detected by high-frequency ultrasound before operation, 3 were misdiagnosed, 45 were negative, and 16 were missed. The sensitivity, specificity, missed diagnosis and misdiagnosis were 94.2%, 90.6%, 5.8% and 9.4%, respectively. The correct rate is 93.8%, the positive likelihood ratio is 10.046, the negative likelihood ratio is 0.064, and the Joden index is 0.848. Conclusion High frequency ultrasound is cheap, safe and efficient. It has high accuracy in detecting foreign bodies in soft tissue. It has important clinical value and can guide the operation.
