不同免疫學(xué)方法對(duì)兔小關(guān)節(jié)異體移植的影響研究

【摘要】:目的:本研究采用多種免疫學(xué)處理方法,對(duì)比研究兔同種異體小關(guān)節(jié)移植的實(shí)驗(yàn)效果,旨在探討一種減少同種異體小關(guān)節(jié)移植免疫排斥反應(yīng),促進(jìn)骨折愈合及避免關(guān)節(jié)退變的有效措施,為臨床上行同種異體小關(guān)節(jié)移植術(shù)提供新途徑。方法:隨機(jī)選取48只成年雄性新西蘭兔,以新西蘭兔上肢第三掌指關(guān)節(jié)為研究對(duì)象,術(shù)前1月處死18只新西蘭兔取第三掌指關(guān)節(jié)進(jìn)行深低溫保存,余下30只隨機(jī)分A、B、C、D、E組,每組實(shí)驗(yàn)動(dòng)物為6只。A組為同種異體關(guān)節(jié)直接移植組,B組為同種異體關(guān)節(jié)直接移植+FK560免疫藥物組,C組為冷凍處理的同種異體關(guān)節(jié)移植組,D組為冷凍處理后的同種異體關(guān)節(jié)移植+FK560免疫藥物組,E組為冷凍處理后的同種異體關(guān)節(jié)移+FK560+MMF+Pred藥物組。異體關(guān)節(jié)移植術(shù)后分別于4周將實(shí)驗(yàn)各組處死1只新西蘭兔觀察其大體標(biāo)本,8周抽取各實(shí)驗(yàn)動(dòng)物組血清進(jìn)行IL-2及IL-6含量檢測(cè),12周進(jìn)行影像學(xué)檢查,48周將實(shí)驗(yàn)動(dòng)物全部處死進(jìn)行常規(guī)HE染色切片來(lái)觀察骨折的愈合以及關(guān)節(jié)面軟骨的情況。對(duì)比各組的實(shí)驗(yàn)結(jié)果,實(shí)驗(yàn)數(shù)據(jù)應(yīng)用SPSS19.0統(tǒng)計(jì)軟件分析處理。結(jié)果:1)大體結(jié)果:術(shù)后4周大體標(biāo)本除A實(shí)驗(yàn)組3只新西蘭兔創(chuàng)口出現(xiàn)不同程度的紅腫、潰爛,其余4組創(chuàng)口均順利愈合。2)術(shù)后8周取兔血清觀察白介素-2結(jié)果:方差分析顯示各組間總差異有統(tǒng)計(jì)學(xué)意義(F=52.163,P0.001)。B、C組與A組比較P0.001,D、E組與B、C組比較P0.001。B與C組之間無(wú)差異(P=0.150),D與E組之間無(wú)差異(P=0.721)。結(jié)果顯示AB、CD、E。術(shù)后8周白介素-6結(jié)果:方差分析顯示各組間總差異有統(tǒng)計(jì)學(xué)意義(F=482.864,P0.001).B、C組與A組比較P0.001,D、E組與B、C組比較P0.001。但B與C組之間無(wú)差異(P=0.164),D與E組之間無(wú)差異(P=0.378)?偨Y(jié)果顯示AB、CD、E。3)術(shù)后12周X線顯示:骨折端骨痂生長(zhǎng)情況及皮質(zhì)骨塑形情況,總體相比B、C、D組結(jié)果均明顯優(yōu)于A.B組。4)術(shù)后48周組織學(xué)觀察關(guān)節(jié)軟骨面Wakitani score評(píng)分:A組得分遠(yuǎn)高于其它組,B、C組間以及D、E組間差異不大?傮w得分為:AB、CD、E。結(jié)論:同種異體小關(guān)節(jié)移植時(shí),供體關(guān)節(jié)術(shù)前經(jīng)深低溫冷凍處理,術(shù)后短期使用免疫抑制藥抗排斥,不僅可以有效的減少免疫排斥反應(yīng),也可以促進(jìn)自體骨與供體骨更好的愈合,而且能延緩移植關(guān)節(jié)遠(yuǎn)期的關(guān)節(jié)退變,從而最大程度的保存異體移植關(guān)節(jié)的功能。
[Abstract]:Objective: to compare the experimental results of rabbit facet allograft transplantation with various immunologic methods in order to explore a method to reduce the immune rejection of allogeneic facet transplantation. The effective measures to promote fracture healing and to avoid joint degeneration provide a new way for clinical transplantation of allogeneic facet. Methods: forty-eight adult male New Zealand rabbits were randomly selected. The third metacarpophalangeal joints of the upper limbs of New Zealand rabbits were killed 1 month before operation for cryopreservation. The remaining 30 rabbits were randomly divided into A, B, C, A, B, C. Group D, Group E, 6 animals in each group. Group A: direct transplantation of joints, Group B: direct transplantation of FK560 immunopharmaceuticals, Group C: cryopreserved allografts, Group A: allografts, Group B: allografts, Group C: cryopreserved allografts. Group D was FK560 immunopharmaceutical group after cryopreservation and group E was cryopreserved allogeneic joint transplantation with FK560 MMF Pred drug group. At 4 weeks after allogeneic joint transplantation, 1 New Zealand rabbit was sacrificed to observe the gross specimen, 8 weeks after transplantation, the serum levels of IL-2 and IL-6 were measured, and imaging examination was carried out at 12 weeks. All the animals were killed at 48 weeks and routine HE staining sections were used to observe the healing of fracture and articular cartilage. Comparing the experimental results of each group, the experimental data were analyzed and processed by SPSS19.0 statistical software. Results: 1) Gross results: 4 weeks after operation, except for 3 New Zealand rabbits in experimental group A, there were different degrees of redness and ulceration in the wound. The results of ANOVA showed that there was a significant difference among the four groups (F = 52.163, P 0.001). B, C group compared with A group P 0.001, D, P < 0.01, P < 0.01, There was no difference between P0.001.B and C (P = 0.150), D vs E, P = 0.721). Results show AB,CD,E. Results of IL-6 at 8 weeks after operation: analysis of variance showed that there was a significant difference among the three groups (F = 482.864, P 0.001). B, C group compared with A group P0.001, D, E group compared with B, C group P 0.001. But there was no difference between group B and C (P = 0.164), D vs group E, P = 0.378). The results showed that 12 weeks after AB,CD,E.3, the growth of callus at fracture end and the shape of cortical bone were observed by X-ray, as compared with those of B, C, C, The results of group D were significantly better than those of group A. 4) the Wakitani score scores of articular cartilage surface in group A were significantly higher than those in other groups at 48 weeks after operation: group B, group C and group D, E had little difference. The overall score is: AB,CD,E. Conclusion: during allogeneic facet transplantation, cryopreservation of donor joints before operation and short-term use of immunosuppressive drugs against rejection after transplantation can not only effectively reduce the immune rejection, but also decrease the rejection of the donor joints. It can also promote the healing of autogenous bone and donor bone, and delay the long-term joint degeneration of transplanted joint, thus preserving the function of allogeneic joint to the maximum extent.
