國人膝關(guān)節(jié)前外側(cè)韌帶的解剖學(xué)研究

【摘要】:目的解剖研究國人膝關(guān)節(jié)前外側(cè)韌帶(anterolateral ligament,ALL),探討ALL結(jié)構(gòu)特征,為臨床上ALL損傷的診治,特別是ALL重建手術(shù)的開展提供參考。方法選用成年尸體膝關(guān)節(jié)標(biāo)本16具,男、女各8具,左、右膝各8具;供體年齡57~84歲,平均73.5歲。根據(jù)標(biāo)準(zhǔn)入路進(jìn)行解剖,并對ALL進(jìn)行定性和定量的描述。結(jié)果 16具標(biāo)本中12具可見ALL,出現(xiàn)率為75.0%。它起自股骨外上髁,起點位于外側(cè)副韌帶(lateral collateral ligament,LCL)的起點近端后方,其走行越過LCL表面或與之并行,韌帶中段與外側(cè)半月板體部邊緣緊密相連,止點位于Gerdy結(jié)節(jié)和腓骨頭之間,止點中心與Gerdy結(jié)節(jié)中心以及腓骨頭最外側(cè)緣的距離分別為(23.9±3.3)、(23.8±4.0)mm。膝關(guān)節(jié)中立位屈曲0°和60°時ALL長度分別為(44.8±5.1)、(47.8±5.5)mm,比較差異無統(tǒng)計學(xué)意義(t=14.071,P=0.000),在膝關(guān)節(jié)屈曲60°合并內(nèi)旋時ALL較緊張。根據(jù)男女進(jìn)行分組比較,屈曲0°和60°時男性ALL長度均顯著大于女性(t=2.920,P=0.015;t=2.806,P=0.019);其余測量指標(biāo)比較,差異無統(tǒng)計學(xué)意義(P0.05)。結(jié)論 ALL是一條位于膝關(guān)節(jié)前外側(cè)的獨立非等長韌帶結(jié)構(gòu),其長度存在性別差異。ALL在維持膝關(guān)節(jié)旋轉(zhuǎn)穩(wěn)定性方面發(fā)揮一定作用,臨床上應(yīng)引起足夠重視。
[Abstract]:Objective to study the anterolateral ligament of knee joint (anterolateral ligament,ALL) in Chinese and to explore the structural characteristics of ALL, and to provide reference for the diagnosis and treatment of ALL injury, especially for the reconstruction of ALL. Methods Sixteen adult cadavers (8 males, 8 females, 8 left and 8 right knees) were selected and the donor age was 5784 years (mean 73.5 years). Dissection was performed according to the standard approach and ALL was described qualitatively and quantitatively. Results the incidence of ALL, was 75.0 in 12 of 16 specimens. It originated from the lateral epicondyle of the femur, and the starting point was behind the starting point of the lateral collateral ligament (lateral collateral ligament,LCL). It crossed or paralleled the surface of the LCL, and the middle part of the ligament was closely connected to the edge of the lateral meniscus. The distance between the Gerdy tubercle and the fibula head, the distance between the center of the stop point and the center of the Gerdy nodule and the most lateral edge of the fibula head is (23.9 鹵3.3), (23.8 鹵4.0) mm., respectively. There was no significant difference in ALL length (44.8 鹵5.1), (47.8 鹵5.5) mm, between 0 擄and 60 擄flexion in neutral position of knee joint (t = 14.071). ALL was more tense when flexion of knee joint was 60 擄with internal rotation. The length of ALL in males was significantly larger than that in females at flexion of 0 擄and 60 擄(t = 2.920, P = 0.015, P = 2.806, P = 0.019), but there was no significant difference in other measurements (P0.05). Conclusion ALL is an independent non-isoscele ligament structure located on the anterolateral side of the knee joint, and its length is different in sex. ALL plays a certain role in maintaining the stability of knee joint rotation, which should be paid more attention in clinic.
