【摘要】：目的探討足背動脈穿支蒂皮瓣修復(fù)前足背側(cè)皮膚軟組織缺損的臨床療效。方法 2011年1月—2016年6月,應(yīng)用足背動脈穿支蒂皮瓣修復(fù)前足背側(cè)皮膚軟組織缺損16例。男10例,女6例;年齡17~65歲,平均28.5歲。致傷原因:重物砸傷9例,交通事故傷7例。左足7例,右足9例。創(chuàng)面均位于跖跗關(guān)節(jié)以遠(yuǎn)前足背側(cè);合并肌腱外露10例,骨外露6例。急癥修復(fù)9例,擇期修復(fù)7例。創(chuàng)面范圍5.0 cm×4.0 cm~7.5 cm×7.0 cm。皮瓣切取范圍為6 cm×6 cm~9 cm×9 cm。結(jié)果 1例皮瓣術(shù)后第2天開始出現(xiàn)張力性水皰,皮瓣腫脹較嚴(yán)重,經(jīng)對癥處理后愈合;其余皮瓣及供區(qū)植皮均成活。術(shù)后16例均獲隨訪,隨訪時間6~18個月,平均8個月。皮瓣顏色、質(zhì)地、厚度均與周圍正常組織相似,患足能正常穿鞋及負(fù)重行走。結(jié)論足背動脈穿支蒂皮瓣手術(shù)操作簡便,無需犧牲主干血管,是修復(fù)前足背側(cè)皮膚軟組織缺損的有效方法之一。
[Abstract]:Objective to investigate the clinical effect of dorsal foot artery perforator pedicle flap in repairing anterior dorsal foot skin and soft tissue defect. Methods from January 2011 to June 2016, 16 cases of anterior dorsalis pedis skin and soft tissue defects were repaired by pedicle flap pedicled with perforating branch of dorsal foot artery. There were 10 males and 6 females, aged 17 to 65 years (mean 28.5 years). The causes of injury included 9 cases of heavy object injury and 7 cases of traffic accident injury. Left foot 7 cases, right foot 9 cases. The wounds were located in the distal dorsalis pedis of tarsometatarsal joint, 10 cases with tendon exposure and 6 cases with bone exposure. Emergency repair in 9 cases, selective repair in 7 cases. Wound area 5.0 cm 脳 4.0 cm~7.5 cm 脳 7.0 cm. The range of flap removal is 6 cm 脳 6 cm~9 cm 脳 9 cm.. Results tension blister appeared on the second day after operation in one case, the swelling of the flap was serious and healed after symptomatic treatment, and the other flaps and donor skin grafts survived. All the 16 cases were followed up for 6 ~ 18 months (mean 8 months). The color, texture and thickness of the flap are similar to those of the surrounding normal tissue. Conclusion the flap pedicled with perforating branch of dorsalis pedis artery is one of the effective methods for repairing skin and soft tissue defect of dorsal foot without sacrificing the main artery.
