【摘要】：目的評(píng)價(jià)血栓彈力圖(thromboelastography,TEG)與常規(guī)檢測(cè)指導(dǎo)心臟手術(shù)輸血對(duì)術(shù)后出血量的影響,為治療決策提供依據(jù)。方法以"Thromboelastography、TEG、血栓彈力圖、血栓彈力描記圖"為檢索詞檢索Pub Med、Embase、The Cochrane Library、中國(guó)生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫(kù)和中國(guó)期刊網(wǎng)全文數(shù)據(jù)庫(kù)建庫(kù)至2015年2月關(guān)于TEG與常規(guī)檢測(cè)指導(dǎo)心臟手術(shù)輸血對(duì)術(shù)后出血量影響的隨機(jī)對(duì)照試驗(yàn)或回顧性隊(duì)列研究,對(duì)符合納入標(biāo)準(zhǔn)的文獻(xiàn)提取資料、評(píng)估方法學(xué)質(zhì)量,并采用Rev Man5.3軟件進(jìn)行meta分析。結(jié)果最終納入7篇文獻(xiàn)共計(jì)689例患者,其中5篇為高質(zhì)量研究,2篇為低質(zhì)量研究。meta分析結(jié)果顯示:TEG與常規(guī)檢測(cè)指導(dǎo)心臟手術(shù)輸血對(duì)術(shù)后出血量的影響差異有統(tǒng)計(jì)學(xué)意義[WMD=-45.67,95%CI(-80.32,-11.03),P0.05]。結(jié)論與常規(guī)檢測(cè)指導(dǎo)心臟手術(shù)輸血比較,TEG指導(dǎo)能明顯減少患者術(shù)后出血量。
[Abstract]:Objective to evaluate the effect of thromboelastography (thromboelastography,TEG) and routine test on postoperative blood loss in cardiac surgery. Methods "Thromboelastography,TEG, thromboelastogram" was used. "Thromboelastography" search Pub Med,Embase,The Cochrane Library, Chinese Biomedical Literature Database and China Journal Web Full-text Database until February 2015 on TEG and routine testing to guide Blood Transfusion in Cardiac surgery for postoperative Hemorrhage Volume Random controlled trials or retrospective cohort studies, The quality of methodology was evaluated and meta analysis was carried out with Rev Man5.3 software. Results A total of 689 patients were included in 7 articles, of which 5 were of high quality and 2 were of low quality. The results of meta analysis showed that there was a significant difference between TEG and routine blood transfusion guidance in cardiac surgery on postoperative bleeding volume [WMD=-45.67,95%CI (-80.32 鹵11.03), P0.05]. Conclusion TEG guidance can significantly reduce postoperative blood loss in patients with cardiac surgery as compared with conventional blood transfusion guidance.
