【摘要】：目的:采用經(jīng)皮內(nèi)鏡椎間孔內(nèi)口入路椎間盤摘除術(shù)(percutanous endoscopic medial foraminal discectomy,PE-MFD)治療腰椎間盤突出癥(lumbar disc herniation,LDH),并探討應(yīng)用PE-MFD技術(shù)治療LDH的安全性和有效性。方法:Ⅰ組32例LDH患者接受了PE-MFD手術(shù),平均隨訪10.1±7.6個(gè)月。分別于術(shù)前、出院前、術(shù)后3~6月及末次隨訪時(shí)行視覺模擬量表(visual analogue scale,VAS)、腰椎疾患量表(jap anese orthopedic association,JOA)、Oswestry功能殘障指數(shù)量表(oswerstry disability index,ODI)評(píng)分,進(jìn)行評(píng)價(jià)療效。同時(shí),對(duì)照Ⅱ組接受TESSYS手術(shù)LDH患者41例,對(duì)比分析手術(shù)及住院期間觀察指標(biāo)。結(jié)果:與治療前相比,PE-MFD術(shù)后及隨訪時(shí)段VAS、JOA及ODI分值均有顯著差異(P0.01)。根據(jù)JOA改善率評(píng)價(jià)療效分級(jí),短期隨訪優(yōu)16例、良14例、一般2例,治療滿意率達(dá)93.75%。兩組未發(fā)生嚴(yán)重手術(shù)并發(fā)癥,在手術(shù)時(shí)間、出血量、自我滿意度、平均住院日等方面,Ⅰ組明顯優(yōu)于Ⅱ組。結(jié)論:PE-MFD治療LDH安全、有效,具有創(chuàng)傷小、對(duì)椎管干擾少、神經(jīng)根減壓充分、易于掌握等特點(diǎn)。
