【摘要】：目的探討改良后清創(chuàng)引流在整形外科慢性潰瘍中的應用。方法選取醫(yī)院整形外科病房2015年10月-2016年3月收治的80例慢性潰瘍創(chuàng)面患者,隨機將手術患者分為研究組和對照組,每組各40例;潰瘍研究組采用負壓封閉引流術(VSD)治療,對照組采用常規(guī)引流,比較兩組患者的日引流量、換藥次數、創(chuàng)面愈合情況、感染控制情況及住院時間。結果研究組的術后日平均引流量為(56.93±10.41)ml多于對照組的(13.65±5.53)ml,差異有統(tǒng)計學意義(t=15.47,P0.05);研究組換藥次數(11.25±2.25)次,少于對照組的(25.64±3.56)次,差異有統(tǒng)計學意義(t=7.51,P0.05);研究組創(chuàng)面愈合時間及住院時間分別為(15.36±1.36)d和(20.56±4.68)d,均短于對照組的(28.56±1.86)d和(30.91±7.82)d,差異有統(tǒng)計學意義(P0.05);研究組感染控制的總有效率為97.5%,對照組感染控制的總有效率為75.0%,兩組比較差異有統(tǒng)計學意義(t=4.9,P0.05)。結論 VSD能夠有效降低整形外科慢性潰瘍患者感染發(fā)生率,加速創(chuàng)面愈合,減輕疼痛,顯著縮短住院時間,降低成本,值得在臨床推廣應用。
[Abstract]:Objective to explore the application of modified debridement and drainage in chronic ulcer in plastic surgery. Methods from October 2015 to March 2016, 80 patients with chronic ulcer wound were randomly divided into study group and control group, 40 patients in each group, the ulcer study group was treated with negative pressure blocking drainage (VSD). The patients in the control group were treated with routine drainage. The daily drainage volume, times of dressing change, wound healing, infection control and hospitalization time were compared between the two groups. Results the average daily drainage volume in the study group was (56.93 鹵10.41) ml, which was significantly higher than that in the control group (13.65 鹵5.53) ml (t = 15.47), and the times of dressing change in the study group was (11.25 鹵2.25) times, less than that in the control group (25.64 鹵3.56) times). The wound healing time and hospitalization time in the study group were (15.36 鹵1.36) days and (20.56 鹵4.68) days, respectively, which were shorter than those in the control group (28.56 鹵1.86) days and (30.91 鹵7.82) days, respectively (P0.05), the total effective rate of infection control in the study group was 97.5d, and the infection control in the control group was (30.91 鹵7.82) days (P0.05). The total effective rate was 75.0, the difference between the two groups was statistically significant (P 0.05). Conclusion VSD can effectively reduce the incidence of infection in patients with chronic ulcer in plastic surgery, accelerate wound healing, relieve pain, shorten the hospitalization time and reduce the cost. It is worth popularizing and applying in clinical practice.
