脾與殘肝體積比和肝切除術(shù)后肝功能相關(guān)分析研究

【摘要】:目的:研究探討脾與殘肝體積比在肝切除手術(shù)后的肝臟儲備功能的評估價值。應(yīng)用肝臟三維重建軟件IQQA-Liver(美國EDDA公司)和水侵法測量原發(fā)性肝癌肝切除術(shù)后殘肝體積、脾臟體積,并計算出脾與殘肝體積比,分析脾與殘肝體積比和肝癌切除術(shù)后肝功能指標的相關(guān)性,了解脾與殘肝體積對術(shù)后肝臟儲備功能的影響,為臨床醫(yī)生在術(shù)前準確評估術(shù)后肝臟功能,避免肝癌切除術(shù)后肝功能衰竭的發(fā)生提供指導(dǎo)。方法:回顧性分析2012年2月至2013年6月共57例被我科診斷為實施手術(shù)治療的肝硬化肝癌患者,應(yīng)用肝臟三維重建軟件IQQA-Liver和水侵法測量原發(fā)性肝癌肝切除術(shù)后殘肝體積、脾臟體積,并計算出脾與殘肝體積比,分析脾與殘肝體積比與患者術(shù)后肝功能情況及住院時間的關(guān)系。結(jié)果:經(jīng)過多元回歸分析結(jié)果提示肝癌肝切除術(shù)后肝功能評分Child-Pugh與脾與殘肝體積比有關(guān)。脾與殘肝體積比值越小。其評分越低(t=7.731,P≈0.000),即肝臟儲備功能越好。當脾與殘肝體積比≤0.9時;颊咧形蛔≡簳r間為13 d(11-15 d)。當脾與殘肝體積比0.9時,患者中位住院時間為21 d(14-28d)。兩者的住院時間差異有統(tǒng)計學(xué)意義(P=0.000)。結(jié)論:脾與殘肝體積比可以有效預(yù)測肝癌肝切除術(shù)后的肝臟儲備功能,術(shù)前應(yīng)制定詳細的手術(shù)計劃,估算殘肝體積(脾與殘肝體積比≤0.9),避免手術(shù)風(fēng)險,減少術(shù)后肝功能損害的發(fā)生。
[Abstract]:Objective: to investigate the value of spleen to residual liver volume ratio in evaluating liver reserve function after hepatectomy. The residual liver volume and spleen volume after hepatectomy were measured by using IQQA-Liver (EDDA company) and water invasion method, and the volume ratio of spleen to remnant liver was calculated. To analyze the correlation between the volume ratio of spleen and remnant liver and the liver function index after hepatectomy, to understand the effect of spleen and residual liver volume on liver reserve function after hepatectomy, so as to evaluate the liver function accurately before and after hepatectomy. To avoid liver failure after hepatectomy provides guidance. Methods: from February 2012 to June 2013, 57 patients with liver cirrhosis diagnosed by our department as surgical treatment were analyzed retrospectively. The residual liver volume after hepatectomy was measured by IQQA-Liver and water invasion software.
