【摘要】：目的探討經(jīng)皮Herbert螺釘微創(chuàng)內(nèi)固定治療新鮮腕舟骨骨折的療效。方法 2012年9月~2016年9月我科采用掌側(cè)經(jīng)皮導(dǎo)針定位Herbert螺釘內(nèi)固定治療11例新鮮腕舟骨骨折(Herbert分型A2型8例,B2型2例,B3型1例)。結(jié)果手術(shù)時(shí)間45~80 min,平均65 min;術(shù)中出血2~30 ml,平均11 ml。術(shù)中透視及術(shù)后X線片均證實(shí)骨折復(fù)位良好,螺釘位置良好,切口術(shù)后2周拆線,均愈合良好。11例隨訪3~18個(gè)月,平均8個(gè)月,骨折均骨性愈合。按改良Mayo腕關(guān)節(jié)功能評(píng)分,優(yōu)9例,良1例,可1例,優(yōu)良率90.9%(10/11)。結(jié)論對(duì)新鮮腕舟骨骨折采用經(jīng)皮閉合復(fù)位Herbert螺釘內(nèi)固定治療效果滿意。
[Abstract]:Objective to evaluate the effect of percutaneous Herbert screw minimally invasive internal fixation for fresh scaphoid fracture. Methods from September 2012 to September 2016, 11 cases of fresh carpal scaphoid fractures were treated with Herbert screw fixation. Results the operative time was 4580 min (mean 65 min) and the intraoperative bleeding was 20 ml (mean 11 ml). Intraoperative fluoroscopy and X ray film showed that the fracture was well reduced and the screw was in good position. All the 11 cases were followed up for 3 ~ 18 months (mean 8 months) at 2 weeks after operation. According to the modified Mayo wrist function score, 9 cases were excellent, 1 case was good, 1 case was fair, and the excellent and good rate was 90.9% 10 / 11%. Conclusion the treatment of fresh scaphoid fracture by percutaneous closed reduction with Herbert screw fixation is satisfactory.
