【摘要】：目的：評(píng)估肩關(guān)節(jié)鏡下單排縫合技術(shù)治療肩袖損傷的效果。方法：選取安徽醫(yī)科大學(xué)第一附屬醫(yī)院骨科運(yùn)動(dòng)創(chuàng)傷與關(guān)節(jié)鏡外科2012年02月-2014年5月40例行肩關(guān)節(jié)鏡下肩袖修補(bǔ)術(shù)患者,40例均獲得術(shù)后隨訪。其中男22例,女18例,年齡37-78歲(平均年齡47歲),右側(cè)肩袖損傷29例,左側(cè)11例。根據(jù)Gerber分型,其中中小型肩袖損傷37例,巨大肩袖損傷3例。手術(shù)均采用金屬縫合錨單排縫合技術(shù)進(jìn)行肩袖修補(bǔ),采用美國肩肘醫(yī)師協(xié)會(huì)評(píng)分(ASES)、Constant-Murley以及美國加州大學(xué)UCLA肩關(guān)節(jié)功能評(píng)分標(biāo)準(zhǔn)進(jìn)行術(shù)前術(shù)后功能評(píng)估。結(jié)果：40例患者術(shù)后均獲得隨訪,隨訪時(shí)間6-31個(gè)月(平均17.3個(gè)月),ASES評(píng)分：40例患者術(shù)前平均為71.9,術(shù)后平均為93.6(P0.001),其中VAS疼痛評(píng)分術(shù)前平均為6.9,術(shù)后平均為1.7(P0.001),肩關(guān)節(jié)前屈上舉術(shù)前平均為76.4,術(shù)后平均為145.6(P0.001),外旋術(shù)前平均為24.3°,術(shù)后平均為37.8°(P0.01)。ASES評(píng)分優(yōu)良占92.5%(37/40)。Constant-Murley評(píng)分術(shù)前為67.4,術(shù)后平均為87.6, Constant-Murley評(píng)分優(yōu)良占90%(36/40)。UCLA評(píng)分術(shù)前平均為15.1,術(shù)后平均為31.2(P0.01),UCLA評(píng)分優(yōu)良占92.5%。結(jié)論：肩關(guān)節(jié)鏡下單排縫合技術(shù)治療肩袖損傷的效果較好,術(shù)中可有效的減少甚至避免血管神經(jīng)的損傷。術(shù)后創(chuàng)傷小、恢復(fù)快,與傳統(tǒng)開放手術(shù)相比患者較容易接受,且手術(shù)并發(fā)癥較少,有效的保護(hù)了維持肩關(guān)節(jié)穩(wěn)定的關(guān)節(jié)囊及肌肉等組織。
[Abstract]:Objective: to evaluate the effect of single-row suture under shoulder arthroscopy in the treatment of rotator cuff injury. Methods: from February 2012 to May 2014, 40 patients underwent arthroscopic rotator cuff repair in the Department of Orthopaedic Trauma and arthroscopy in the first affiliated Hospital of Anhui Medical University were followed up. There were 22 males and 18 females aged 37-78 years (mean age 47 years), 29 cases with right rotator cuff injury and 11 cases with left rotator cuff injury. According to Gerber classification, there were 37 cases of small rotator cuff injury and 3 cases of giant rotator cuff injury. Metal suture anchor single row suture technique was used to repair the rotator cuff. ASESU Constant-Murley scale and UCLA UCLA function scale were used to evaluate the preoperative and postoperative function. Results all 40 patients were followed up after operation. The follow-up time was 6-31 months (mean 17.3 months). The average preoperative score was 71.9, and the postoperative average was 93.6p 0.001. The average VAS pain score was 6.9 before, 1.7 P 0.001 after operation, 76.4 before and after the shoulder flexion. The average score was 24.3 擄before external rotation, 37.8 擄before operation, 37.8 擄P 0.01U 路ASES, 92.5% 37 / 40%. Constant-Murley score was 67.4 before operation, 87.6 after operation, excellent Constant-Murley score accounted for 15.1% of 90 cases before operation, and 31.2 擄P 0.01 after operation, and 92.5% with good UCLA score. Conclusion: single row suture under shoulder arthroscopy is effective in the treatment of rotator cuff injury, which can effectively reduce or even avoid the injury of blood vessel and nerve. Compared with the traditional open surgery, the patients were more easily accepted and had fewer complications, which effectively protected the joint capsule and muscle tissue which maintained the stability of the shoulder joint.
