【摘要】：目的:探究及對比內(nèi)鏡下置入經(jīng)鼻腸梗阻導(dǎo)管和普通鼻胃管在治療粘連性腸梗阻中的有效性和推廣可行性。方法:收集2014年9月-2016年11月山西醫(yī)科大學(xué)第一醫(yī)院、第二醫(yī)院及省人民醫(yī)院治療的95例確診為粘連性腸梗阻的患者。隨機將患者分為研究組和對照組進行減壓治療,研究組是在內(nèi)鏡操作下置入經(jīng)鼻腸梗阻導(dǎo)管的50例患者;對照組是應(yīng)用普通鼻胃管行減壓治療的45例患者。分別比較兩組患者治療效果。結(jié)果:應(yīng)用經(jīng)鼻腸梗阻導(dǎo)管治療的研究組患者與應(yīng)用普通鼻胃管治療的對照組患者相比較,在腸梗阻癥狀減輕時間、腹部影像學(xué)特征改善的時間(4.4±2.35d vs 8.6±4.7d)上顯著縮短,具有統(tǒng)計學(xué)差異,P0.01。保守治療成功率相比較,研究組為92.0%,對照組為46.7%,P0.01。在研究組中有8%的患者保守治療失敗行手術(shù)治療;對照組中有53.3%的患者最終行手術(shù)治療。既往確診為粘連性腸梗阻的再次復(fù)發(fā)的患者,經(jīng)鼻腸梗阻導(dǎo)管在治療效果上較普通鼻胃管顯著(95.0%vs 37.5%)。在經(jīng)鼻腸梗阻導(dǎo)管治療組中,患者保守治療的是否成功與患者第一天的胃腸減壓量及腹部影像學(xué)的改變相關(guān),P0.05。結(jié)論:1、經(jīng)鼻腸梗阻導(dǎo)管在臨床上可應(yīng)用于粘連性腸梗阻的治療,而且治療效果顯著優(yōu)于普通鼻胃管。2、經(jīng)鼻腸梗阻導(dǎo)管在治療粘連性腸梗阻中,根據(jù)胃腸減壓量及腹部影像學(xué)改變情況,預(yù)測保守治療成功率及為手術(shù)時機提供參考。3、即使存在一些潛在危險性和并發(fā)癥,但是經(jīng)內(nèi)鏡下置入經(jīng)鼻腸梗阻導(dǎo)管較為簡單可行,在臨床上可大量推廣使用。
[Abstract]:Objective: To explore and compare the effectiveness and popularization feasibility of endoscopic sinus obstruction catheter and ordinary nasogastric tube in the treatment of adhesive intestinal obstruction. Methods: 95 patients with adhesive intestinal obstruction treated in the first hospital of Shanxi Medical University, second hospital and the provincial people's Hospital in September 2014, -2016, were randomly selected. The study group was divided into the study group and the control group for the decompression treatment. The study group was placed in 50 patients with endoscopic sinus obstruction catheterization; the control group was 45 patients with normal nasal gastric tube decompression treatment. The results were compared with the two groups of patients. Results: patients in the study group with the nasal intestinal obstruction catheter treatment and the application of the ordinary nose.
