【摘要】：[目的]探討富血小板血漿(PRP)與萬(wàn)向空心釘鎖定鋼板治療股骨頸骨折的臨床療效。[方法]2012年1月~2015年1月本科收治60例股骨頸骨折患者,以單盲法按數(shù)字隨機(jī)分為PRP聯(lián)合萬(wàn)向空心釘鎖定鋼板治療組(聯(lián)合組)、單純?nèi)f向空心釘鎖定鋼板治療組(單純組),每組30例。對(duì)比兩組患者手術(shù)時(shí)間、術(shù)中出血量、骨折愈合時(shí)間、完全負(fù)重所需時(shí)間、髖關(guān)節(jié)功能Haris評(píng)分、股骨頭壞死發(fā)生率。[結(jié)果]兩組患者手術(shù)時(shí)間、術(shù)中出血量比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)�；颊咂骄S訪時(shí)間(18.82±4.21)個(gè)月,在完全負(fù)重所需時(shí)間[(10.32±1.84)個(gè)月vs(12.83±2.25),P0.001],骨折愈合時(shí)間[(9.47±2.64)個(gè)月vs(12.76±2.35)個(gè)月,P0.001],髖關(guān)節(jié)功能Harris評(píng)分[(90.12±5.34)vs(78.43±4.14),P0.001]方面聯(lián)合組均顯著優(yōu)于單純組,差異有統(tǒng)計(jì)學(xué)意義。聯(lián)合組僅有1例患者發(fā)生股骨頭壞死,而單純組有3例發(fā)生股骨頭壞死,但兩組差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。[結(jié)論]PRP聯(lián)合萬(wàn)向空心釘鎖定板治療青壯年股骨頸骨折可促進(jìn)骨折愈合,具有固定牢靠、手術(shù)創(chuàng)傷小等優(yōu)點(diǎn)。
[Abstract]:Objective: to investigate the clinical effect of platelet rich plasma (PRP) and universal hollow nail locking plate in the treatment of femoral neck fracture. [methods] from January 2012 to January 2015, 60 cases of femoral neck fracture were treated. According to the single blind method, the patients were randomly divided into PRP combined with universal hollow nail locking plate group (combined group) and simple universal hollow nail locking plate group (30 cases in each group). The operative time, intraoperative blood loss, fracture healing time, total weight bearing time, hip function Haris score, and the incidence of femoral head necrosis were compared between the two groups. [results] there was no significant difference in operative time and intraoperative bleeding between the two groups (P 0.05). The average follow-up time was 18.82 鹵4.21 months, the time of complete weight loading was 10.32 鹵1.84 months (vs(12.83 鹵2.25) P0.001, the time of fracture healing was 9.47 鹵2.64 months (vs(12.76 鹵2.35) months P0.001, and the Harris score of hip joint function [90.12 鹵5.34)vs(78.43 鹵4.14P0.001] was significantly better than that of simple group (P 0.001). There was only one case of femoral head necrosis in the combined group and 3 cases in the simple group, but the difference between the two groups was not statistically significant (P 0.05). [conclusion] PRP combined with universal hollow nail locking plate in the treatment of femoral neck fracture in young adults can promote fracture healing, have the advantages of firm fixation and less surgical trauma.
