鞘內注射右美托咪啶對大鼠急性疼痛行為及脊髓背角c-Fos蛋白表達的影響

【摘要】:目的:觀察鞘內注射右美托咪定對切口痛模型大鼠急性疼痛行為學指標的影響,并觀察其對脊髓背角c-Fos蛋白表達的影響,探討其可能的作用機制,為術后疼痛的防治提供有效的臨床研究及理論依據。方法:本實驗共分為兩部分:(1)鞘內注射右美托咪定對切口痛模型大鼠急性疼痛行為學指標的影響。(2)鞘內注射右美托咪定對切口痛模型大鼠血清炎性細胞因子(TNF-α、IL-6和IL-10)及脊髓背角c-Fos蛋白表達的影響。選擇健康成年雄性SD大鼠96只,體重250~300g,采用隨機數(shù)字表法,將其隨機分為4組,每組24只。正常對照組(C組):不做任何處理;手術處理組(O組):行右足切開手術;生理鹽水+手術處理組(N組):鞘內注射生理鹽水20μl后行右足切開手術;右美托咪定+手術處理組(D組):鞘內注射右美托咪定1ug/kg(容量為20μl)后行右足切開手術。經上述處理后,分別于術前(T0)、術后1h、2h、4h、8h、12h、24h及48h時(T1~T7)測定大鼠右足急性疼痛行為學指標[機械縮足反應閾值(MWT)和熱縮足潛伏期(TWL)];并分別于上述時點經腹主動脈采集血樣,采用Elisa法檢測血清TNF-α、IL-6以及IL-10的濃度;采血結束后處死大鼠,灌注4%多聚甲醛,取L4-6脊髓段,采用免疫組織化學法,檢測鞘內注射右美托咪定對切口痛模型大鼠脊髓背角c-Fos蛋白表達的影響。結果:(1)與C組比較,O組、N組和D組大鼠術后機械縮足反應閾值(MWT)降低,熱縮足潛伏期(TWL)縮短(P0.05);與O組比較,D組大鼠術后機械縮足反應閾值(MWT)升高,熱縮足潛伏期(TWL)延長(P0.05),N組差異無統(tǒng)計學意義(P0.05);與N組比較,D組大鼠術后機械縮足反應閾值(MWT)升高,熱縮足潛伏期(TWL)延長(P0.05)。(2)與C組比較,O組、N組和D組大鼠術后血清TNF-α、IL-6以及IL-10濃度升高(P0.05);與O組比較,D組大鼠術后血清TNF-α及IL-6濃度降低,IL-10濃度升高(P0.05),N組差異無統(tǒng)計學意義(P0.05);與N組比較,D組大鼠術后血清TNF-α及IL-6濃度降低,IL-10濃度升高(P0.05)。(3)與C組比較,O組、N組和D組大鼠術后脊髓背角c-Fos蛋白表達明顯升高(P0.05);與O組比較,D組大鼠脊髓背角c-Fos蛋白表達明顯降低(P0.05),N組差異無統(tǒng)計學意義(P0.05);與N組比較,D組大鼠脊髓背角c-Fos蛋白表達明顯降低(P0.05)。結論:鞘內預先注射右美托咪定能顯著提高切口痛模型大鼠機械痛閾值,減輕大鼠急性疼痛行為,其機制可能與其降低炎性反應及大鼠脊髓背角c-Fos蛋白表達有關。
[Abstract]:Objective: to observe the effect of intrathecal injection of dexmetomidine on the behavioral parameters of acute pain in rats with incision pain, and to observe the effect of dexmetidine on the expression of c-Fos protein in spinal dorsal horn, and to explore its possible mechanism. To provide effective clinical research and theoretical basis for the prevention and treatment of postoperative pain. Methods: this experiment was divided into two parts: (1) Intrathecal injection of dexmetomidine on acute behavioral index of pain induced by incision pain in rats. (2) intrathecal injection of dexmetomidine on serum inflammatory cytokine TNF- 偽 IL-6 and IL-6 in rats with incision pain IL-10) and the expression of c-Fos protein in spinal dorsal horn. 96 male Sprague-Dawley (SD) rats were randomly divided into 4 groups with 24 rats in each group. Normal control group C: no treatment, operation group O: right foot incision, saline operation group: intrathecal injection of normal saline 20 渭 l, right foot incision; Group D: right foot incision was performed after intrathecal injection of dexmetomidine at a dose of 20 渭 l. After the above treatment, the behavioral indexes of acute pain in right foot of rats were measured before operation, 1 h, 2 h, 4 h, 8 h, 12 h, 24 h and 48 h, respectively. Blood samples were collected from abdominal aorta of rats at the same time points, including mechanical foot contraction threshold (MWTs) and thermal contraction latency (TWL). Serum TNF- 偽 IL-6 and IL-10 levels were detected by Elisa method, the rats were killed after blood collection, 4% paraformaldehyde was infused, L4-6 spinal cord segment was taken out, and immunohistochemical method was used. To investigate the effect of intrathecal injection of dexmetomidine on the expression of c-Fos in spinal dorsal horn of rats with incision pain. Results compared with group C, the MWTs of rats in group O and group D were decreased, and the latency of thermal contraction was shortened (P 0.05), and the threshold of mechanical contraction in group D was higher than that in group O. There was no significant difference between group N and group N in prolonging the latent period of thermal contraction (TWL), and the threshold of mechanical foot contraction in group D was higher than that in group N. Compared with group C, the levels of TNF- 偽 IL-6 and IL-10 were increased in group N and group D, but there was no significant difference between group D and group D in decreasing serum concentration of TNF- 偽 and IL-6 and increasing concentration of IL-10. Compared with group N and group C, the expression of c-Fos protein in spinal dorsal horn of rats in group D and group D was significantly higher than that in group C and group O, respectively, and the expression of c-Fos in group D was significantly higher than that in group O, compared with group O, the expression of c-Fos in the spinal cord of group D was significantly higher than that in group D, and that in group D was significantly higher than that in group C (P 0.05), and the expression of c-Fos in group D was significantly higher than that in group C (P 0.05). There was no significant difference in the expression of c-Fos protein between the two groups, and the expression of c-Fos protein in the spinal dorsal horn of the D group was significantly lower than that of the N group. Conclusion: intrathecal injection of dexmetidine can significantly increase the mechanical pain threshold and alleviate the acute pain behavior of rats with incision pain. The mechanism may be related to the decrease of inflammatory response and the expression of c-Fos protein in the dorsal horn of spinal cord in rats.
