【摘要】：目的:探討應(yīng)用矩形推進(jìn)皮瓣修復(fù)兒童足跟部皮膚缺損的方法及療效。方法:自2014年5月至2015年6月,共收治7例兒童跟腱周圍皮膚缺損的病例,皮膚缺損面積為2.7cm×3.4cm—4.0cm×5.5cm,有1例在跟腱止點處部分?jǐn)嗔?其余6例跟腱保持完整。清創(chuàng)后采用自行設(shè)計的矩形推進(jìn)皮瓣修復(fù)足跟部皮膚缺損。跟腱部分?jǐn)嗔颜?先將有污染的殘端切除,再閉合傷口。術(shù)后為降低皮瓣的張力,用1枚1.5mm克氏針將踝關(guān)節(jié)固定于跖屈位并用石膏托保護(hù)。結(jié)果:皮瓣全部成活,有2例傷口局部感染,經(jīng)換藥后愈合,其余傷口愈合良好。隨訪時間8-12個月(平均11.5個月),有2例患兒傷口瘢痕較明顯,其余患兒皮瓣色澤、質(zhì)地與周圍組織接近,踝關(guān)節(jié)功能皆正常,治療效果滿意。結(jié)論:矩形推進(jìn)皮瓣操作簡單,安全可靠,是修復(fù)兒童跟腱周圍皮膚缺損的有效方法。
[Abstract]:Objective: to explore the method and effect of rectangular thrust skin flap for repairing heel skin defect in children. Methods: from May 2014 to June 2015, a total of 7 children with skin defects around Achilles tendon were treated. The area of skin defect was 2.7cm 脳 3.4cm-4.0cm 脳 5.5 cm. There was a partial rupture at the end of Achilles tendon in 1 case, and the remaining 6 cases remained intact. After debridement, the skin defect of heel was repaired with self-designed rectangular propulsion flap. In the case of partial rupture of the Achilles tendon, the contaminated stump is removed and then the wound is closed. In order to reduce the tension of the flap, a 1.5mm Kirschner pin was used to fix the ankle joint to the metatarsal flexion and to protect it with plaster. Results: all the flaps survived. Two cases of local infection healed after dressing change. The other wounds healed well. The follow-up time was 8 to 12 months (mean 11.5 months). There were 2 cases with obvious scar, the color and texture of the skin flap were close to the surrounding tissue, the function of ankle joint was normal, and the treatment effect was satisfactory. Conclusion: rectangular thrust flap is an effective method for repairing skin defect around Achilles tendon in children.
