【摘要】：【目的】探討Ribbed解剖型生物固定股骨柄假體(Ribbed柄)的設(shè)計(jì)特征及其與臨床療效關(guān)系。【方法】對2010年3月至2012年3月因髖關(guān)節(jié)疾病,在我院采用Ribbed柄行全髖關(guān)節(jié)置換(THA)術(shù)的患者進(jìn)行回顧性分析。共獲完整資料者52例(52髖),男20例,女32例;手術(shù)時年齡22~78歲,平均59歲。隨訪時間2~4年,平均3.1年。評估手術(shù)前后Harris評分、術(shù)后大腿疼痛率、術(shù)中骨折發(fā)生率、切口愈合情況、脫位及翻修等。影像學(xué)重點(diǎn)評估股骨柄髓腔充填率、股骨柄假體下沉、假體周圍放射學(xué)透亮線、骨長入、應(yīng)力遮擋及骨溶解等�！窘Y(jié)果】術(shù)中股骨骨折發(fā)生率5.8%;所有切口均一期愈合;無髖關(guān)節(jié)脫位及翻修病例;大腿疼痛率為5.8%,均于術(shù)后1年消失;末次隨訪時,平均Harris評分由術(shù)前48分提高到末次隨訪時96分。術(shù)后X線片示所有患者股骨髓腔充填滿意,平均充填率:正位片上干骺端、中段和遠(yuǎn)端分別為91%、88%和86%,側(cè)位片上干骺端、中段和遠(yuǎn)端分別為88%、85%和81%;末次隨訪時,49髖(92.4%)為穩(wěn)定性骨長入固定,3髖為穩(wěn)定性纖維長入固定,股骨側(cè)及髖臼側(cè)假體周圍均未見骨溶解;5髖(9.6%)發(fā)生股骨柄假體下沉,但均2 mm;所有患者均出現(xiàn)不同程度的應(yīng)力遮擋現(xiàn)象,其中1度31髖、2度19髖、3度2髖,無4度應(yīng)力遮擋發(fā)生。【結(jié)論】Ribbed柄設(shè)計(jì)符合生物學(xué)固定要求,術(shù)后能夠獲得生物學(xué)骨長入穩(wěn)定,可取得滿意的臨床療效和影像學(xué)結(jié)果。
[Abstract]:[Objective] to investigate the Ribbed anatomical biological fixation of femoral stem prosthesis (Ribbed handle) the design features and its relationship with clinical efficacy. [method] on March 2010 to March 2012 for hip disease in our hospital with Ribbed handle for total hip arthroplasty (THA) patients were retrospectively analyzed. 52 cases received complete data (52 hips), male 20 cases, female 32 cases; surgery at age 22~78 years old, average 59 years old.
