【摘要】：風濕性心臟瓣膜病是目前發(fā)展中國家最常見的導致年輕人死亡的心血管疾病之一,風濕熱引起的瓣膜病變中以二尖瓣最常受累。手術治療是目前臨床上最有效的治療方法。與傳統(tǒng)正中開胸手術相比,微創(chuàng)心臟手術擁有明顯較小的術中創(chuàng)傷,術后低并發(fā)癥的發(fā)生,患者更少住院天數(shù),在外觀上更能被患者所接受等優(yōu)點,越來越流行,并有可能替代傳統(tǒng)正中開胸方式。本綜述通過闡述微創(chuàng)二尖瓣手術的發(fā)展歷程,并分別闡述胸骨部分切開手術入路、右側(cè)胸壁小切口入路、胸腔鏡輔助微創(chuàng)心臟手術及機器人輔助微創(chuàng)心臟手術這4種不同入路方式微創(chuàng)二尖瓣手術的安全性、可行性、療效及預后等,探討未來微創(chuàng)二尖瓣手術的發(fā)展趨勢及前景。
[Abstract]:Rheumatic valvular disease is one of the most common cardiovascular diseases leading to death in young people in developing countries, and mitral valve is the most frequently involved in valvular disease caused by rheumatic fever.Surgical treatment is the most effective method in clinic.Compared with the traditional median thoracotomy, minimally invasive cardiac surgery has the advantages of less trauma, lower postoperative complications, fewer hospital days, more acceptable to patients in appearance, and more and more popular.And it is possible to replace the traditional central thoracotomy.This article reviews the development of minimally invasive mitral valve surgery, and describes the approach of partial sternotomy and small incision of right chest wall, respectively.The safety, feasibility, curative effect and prognosis of minimally invasive mitral valve surgery by thoracoscopic assisted minimally invasive cardiac surgery and robot-assisted minimally invasive cardiac surgery were studied. The development trend and prospect of minimally invasive mitral valve surgery in the future were discussed.
