【摘要】：目的:研究水飛薊賓對行肝切除術(shù)后患者的肝臟臨床保護作用。方法:對某院行肝切除術(shù)的患者隨機分為觀察組和對照組,其中觀察組患者于實驗前一個星期給予水飛薊賓膠囊,術(shù)后靜脈滴注還原型谷胱甘肽和異甘草酸鎂注射液治療,對照組患者僅于術(shù)后靜脈滴注還原型谷胱甘肽和異甘草酸鎂注射液治療,術(shù)后7 d內(nèi)觀察記錄患者的肝衰竭和肝功能不足發(fā)生情況,于術(shù)前及術(shù)后1,3,5 d和7 d,測定患者血清中總膽紅素(TBIL)、直接膽紅素(DBIL)、谷丙轉(zhuǎn)氨酶(ALT)和谷草轉(zhuǎn)氨酶(AST)水平,評估水飛薊賓膠囊對術(shù)后患者肝臟保護作用。結(jié)果:觀察組肝衰竭和肝功能不足發(fā)生率顯著低于對照組(χ~2=6.202,P=0.0130.05;χ~2=4.579,P=0.0320.05);術(shù)后1 d,觀察組的ALT水平為(287.5±115.8)U·L~(-1),AST水平為(215.7±121.4)U·L~(-1),TBIL水平為(23.3±9.7)μmol·L~(-1),DBIL水平為(8.6±5.1)μmol·L~(-1),對照組的ALT(387.5±156.9)U·L~(-1),AST水平為(316.8±176.4)U·L~(-1),TBIL水平為(27.4±10.3)μmol·L~(-1),DBIL水平為(13.2±5.3)μmol·L~(-1),2組相比,差異有統(tǒng)計學(xué)意義(P0.05)。術(shù)后7 d,觀察組患者的ALT水平為(41.4±11.5)U·L~(-1),AST水平為(31.9±6.3)U·L~(-1),TBIL水平為(5.5±5.4)μmol·L~(-1),DBIL水平為(4.2±3.4)μmol·L~(-1),對照組患者的ALT水平為(114.7±35.2)U·L~(-1),AST水平為(98.5±13.7)U·L~(-1),TBIL水平為(14.3±5.8)μmol·L~(-1),DBIL水平為(7.2±3.5)μmol·L~(-1),2組相比,差異有統(tǒng)計學(xué)意義(P0.05)。結(jié)論:術(shù)前應(yīng)用水飛薊賓膠囊,可有效減輕手術(shù)對肝功能的影響;術(shù)后應(yīng)用水飛薊賓膠囊治療,可以顯著恢復(fù)患者肝功能。
[Abstract]:Objective: to study the clinical protective effect of silybin on liver after hepatectomy.Methods: patients undergoing hepatectomy in a certain hospital were randomly divided into observation group and control group. The patients in the observation group were given silybin capsule one week before the experiment and were treated by intravenous drip of glutathione and magnesium isoglycyrrhizinate injection after operation.The patients in the control group were treated with reduced glutathione and magnesium isoglycyrrhizinate only by intravenous drip. The incidence of liver failure and hepatic insufficiency were observed and recorded within 7 days after operation.The serum levels of Tbilirubin, direct bilirubin, alanine aminotransferase (alt) and aspartate aminotransferase (AST) were measured before and 1 day after operation.
