【摘要】：目的探討頸椎間盤(pán)退變與吸煙的關(guān)系。方法選取2014年6月—2016年6月同濟(jì)大學(xué)附屬東方醫(yī)院脊柱外科門(mén)診因頸肩部疼痛就診的患者320例,根據(jù)煙草燃燒物暴露方式分為非吸煙組(n=141)、被動(dòng)吸煙組(n=55)、吸煙組(n=124)。納入患者均接受MRI檢查,采用Miyazaki分級(jí)評(píng)價(jià)患者頸椎間盤(pán)退變程度,采用視覺(jué)模擬評(píng)分量表(VAS)評(píng)價(jià)患者頸肩痛程度。根據(jù)3組患者M(jìn)iyazaki分級(jí)和VAS評(píng)分探討頸椎間盤(pán)退變與吸煙的關(guān)系。結(jié)果非吸煙組患者C2/3、C3/4、C5/6節(jié)段及總體Miyazaki分級(jí)優(yōu)于被動(dòng)吸煙組,非吸煙組患者C1/2~C6/7節(jié)段及總體Miyazaki分級(jí)優(yōu)于吸煙組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。被動(dòng)吸煙組、吸煙組中,C4/5~C6/7節(jié)段Miyazaki分級(jí)為Ⅳ、Ⅴ級(jí)的椎間盤(pán)比例高于C1/2~C3/4節(jié)段,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。吸煙組中,男性Miyazaki分級(jí)為Ⅳ、Ⅴ級(jí)的椎間盤(pán)比例高于女性,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。被動(dòng)吸煙組、吸煙組中,吸煙5~10年者M(jìn)iyazaki分級(jí)優(yōu)于吸煙10年者,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。3組患者VAS評(píng)分有差異,其中吸煙組患者VAS評(píng)分高于非吸煙組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論煙草燃燒物可加速頸椎間盤(pán)退變進(jìn)程,同時(shí)患者伴隨的頸肩痛程度也有所增加;在影響椎間盤(pán)退變的程度上,下頸椎間盤(pán)較上頸椎間盤(pán)明顯。
[Abstract]:Objective to investigate the relationship between cervical disc degeneration and smoking.Methods 320 patients with neck and shoulder pain were selected from the Department of Spinal surgery, Dongfang Hospital affiliated to Tongji University from June 2014 to June 2016. The patients were divided into non-smoking group (n = 141), passive smoking group (n = 55) and smoking group (n = 124).All the patients were examined by MRI, the degree of cervical disc degeneration was evaluated by Miyazaki grade, and the degree of neck and shoulder pain was evaluated by visual analogue scale (VAS).The relationship between cervical disc degeneration and smoking was studied according to Miyazaki grade and VAS score.Results the C _ 2 / 3 / C _ 3 / C _ 3 / 4 C _ 5 / 6 segment and total Miyazaki grade in non-smoking group were better than those in passive smoking group. The C1/2~C6/7 segment and total Miyazaki grade in non-smoking group were better than that in smoking group (P < 0.05).In the passive smoking group, the proportion of intervertebral disc in C4 / 5 / C6 / 7 segment with Miyazaki grade 鈪，
