單髁關(guān)節(jié)置換的臨床應(yīng)用:最好技術(shù)與最好療效

【摘要】:背景:隨著外科技術(shù)的迅速發(fā)展,單髁關(guān)節(jié)置換已經(jīng)成為骨性關(guān)節(jié)炎外科治療的熱點(diǎn)之一,其治療骨性關(guān)節(jié)炎時(shí)在創(chuàng)傷大小、術(shù)后康復(fù)速度等方面取得了顯著的進(jìn)展。目的:綜述單髁關(guān)節(jié)置換的發(fā)展歷史、臨床療效,并聚焦外側(cè)單髁關(guān)節(jié)置換和計(jì)算機(jī)導(dǎo)航及機(jī)器人輔助的研究進(jìn)展,為單髁關(guān)節(jié)置換進(jìn)一步臨床研究提供參考。方法:以"Unicompartmental knee arthroplasty,Osteoarthritis,Unilateral compartment Lateral knee arthroplasty"為關(guān)鍵詞檢索1955年1月至2017年4月Pub Med數(shù)據(jù)庫(kù)及Web of science數(shù)據(jù)庫(kù)。結(jié)果與結(jié)論:共檢索到文獻(xiàn)562篇,進(jìn)行初步篩選,排除研究目的與本文無(wú)關(guān)及重復(fù)研究,共納入75篇英文文獻(xiàn)進(jìn)行綜述。隨著單間室骨性關(guān)節(jié)炎治療方法的不斷改進(jìn),單髁關(guān)節(jié)置換作為單間室骨性關(guān)節(jié)炎的治療方法,可以保留交叉韌帶、保留更多的骨量,嚴(yán)格的把握單髁關(guān)節(jié)置換的手術(shù)指征,合理的實(shí)施術(shù)后功能鍛煉,對(duì)于獲得最佳的臨床療效至關(guān)重要。同時(shí)計(jì)算機(jī)導(dǎo)航及機(jī)器人輔助技術(shù)的發(fā)展,為單間室骨性關(guān)節(jié)炎的外科治療開辟了新的思路,使得單髁關(guān)節(jié)置換更加精準(zhǔn)和微創(chuàng),但新技術(shù)的應(yīng)用能否進(jìn)一步提高臨床療效仍需要進(jìn)一步的研究證實(shí)。
[Abstract]:Background: with the rapid development of surgical techniques, single condylar arthroplasty has become one of the hot points in the surgical treatment of osteoarthritis. Significant progress has been made in the speed of rehabilitation after operation. Objective: to review the history and clinical effect of monondylar arthroplasty, and focus on the research progress of lateral monondylar arthroplasty, computer navigation and robot-assisted arthroplasty. Methods: to search Pub Med database and Web of science database from January 1955 to April 2017 with "Unicompartmental knee arthroplasty Lateral knee arthroplasty". Results and conclusion: a total of 562 articles were retrieved. The primary screening was carried out, the purpose of the study was excluded from this article and repeated studies were excluded, and 75 articles of English literature were reviewed. As the treatment of single chamber osteoarthritis continued to improve, Single condylar joint replacement as a single chamber osteoarthritis treatment method, can preserve the cruciate ligament, retain more bone, strictly grasp the single condylar joint replacement surgery indications, reasonable implementation of postoperative functional exercise. At the same time, the development of computer navigation and robot-assisted technology has opened up new ideas for the surgical treatment of single chamber osteoarthritis, which makes single condylar joint replacement more accurate and minimally invasive. However, whether the application of new technology can further improve the clinical efficacy still needs further research and confirmation.
