重復(fù)經(jīng)顱磁刺激對脊髓損傷后神經(jīng)性疼痛及大腦皮質(zhì)興奮性的影響

【摘要】:目的:觀察重復(fù)經(jīng)顱磁刺激(rTMS)對脊髓損傷后神經(jīng)性疼痛(NP)的治療效果及其對脊髓損傷后大腦皮質(zhì)興奮性的影響。方法:本研究選取在我科住院治療的不完全性脊髓損傷后神經(jīng)性疼痛患者共32例,用隨機數(shù)字表法將32例患者分為實驗組和對照組,試驗組(n=17)給予常規(guī)物理治療及右側(cè)大腦M1區(qū)rTMS治療,對照組(n=15)給予常規(guī)物理治療及右側(cè)大腦M1區(qū)假刺激。兩組治療均為每天1次,每周6天,連續(xù)治療4周。于治療前、治療4周后對兩組患者進(jìn)行視覺模擬評分(VAS)測試,并對右側(cè)大腦半球的靜息運動閾值(RMT)、運動誘發(fā)電位(MEP)進(jìn)行測試及分析。結(jié)果:治療4周后,試驗組VAS評分降低,RMT波幅降低、MEP波幅增高,與對照組比較有顯著性差異(P0.05)。結(jié)論:rTMS能有效緩解脊髓損傷后神經(jīng)性疼痛,其機制可能與大腦皮質(zhì)興奮性改變相關(guān)。
[Abstract]:Objective: to observe the therapeutic effect of repetitive transcranial magnetic stimulation (RTMS) on neuropathic pain after spinal cord injury (NPN) and its effect on the excitability of cerebral cortex after spinal cord injury. There were 32 cases of neuropathic pain after medullary injury. 32 patients were randomly divided into experimental group and control group. The experimental group was treated with routine physical therapy and the right M1 region of the brain was treated with rTMS. The control group was treated with routine physical therapy and false stimulation in M1 region of the right brain. The two groups were treated once a day, 6 days a week for 4 weeks. Before and after 4 weeks of treatment, the visual analogue scores (VASs) were measured in both groups. The resting exercise threshold (RMT) and motor evoked potential (MEP) of the right hemisphere were measured and analyzed. Results: after 4 weeks of treatment, the VAS score decreased and the amplitude of VAS decreased and the amplitude of MEP increased in the experimental group. Compared with the control group, there was a significant difference between the two groups (P0.050.Conclusion: RTMS can effectively relieve the neuralgia after spinal cord injury, and its mechanism may be related to the changes of cerebral cortex excitability.
