【摘要】：[目的]比較解剖鋼板與鎖定鋼板內(nèi)固定治療復(fù)雜脛骨平臺(tái)骨折的臨床效果。[方法]將2010年1月~2013年12月在本科治療的78例復(fù)雜脛骨平臺(tái)骨折患者隨機(jī)數(shù)字表法分為2組。解剖鋼板組(39例)給予解剖鋼板內(nèi)固定治療,鎖定鋼板組(39例)給予鎖定鋼板內(nèi)固定治療。統(tǒng)計(jì)兩組圍術(shù)期指標(biāo)(手術(shù)時(shí)間、術(shù)中出血量、住院時(shí)間、骨折愈合時(shí)間、完全負(fù)重時(shí)間)、脛骨平臺(tái)畸形[內(nèi)反角(TPA)、后傾角(PA)]、HSS評(píng)分及恢復(fù)情況。[結(jié)果]兩組手術(shù)時(shí)間、術(shù)中出血量、住院時(shí)間、骨折愈合時(shí)間、完全負(fù)重時(shí)間比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)�；颊唠S訪1~2年,兩組術(shù)后即刻、術(shù)后2個(gè)月、術(shù)后6個(gè)月時(shí)TPA、PA比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。兩組術(shù)后1年時(shí)HSS評(píng)分比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。兩組關(guān)節(jié)功能恢復(fù)比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。[結(jié)論]解剖鋼板與鎖定鋼板內(nèi)固定治療復(fù)雜脛骨平臺(tái)骨折的臨床效果相當(dāng),臨床應(yīng)依據(jù)患者具體情況合理選擇術(shù)式。
[Abstract]:[Objective] compared with anatomical plate and the January 2010 ~2013 year in December in our treatment of 78 cases of complex tibial plateau fractures were randomly divided into 2 groups. Methods the clinical effect of] locking plate fixation of complex tibial plateau fracture. The anatomical plate group (39 cases) with anatomic steel plate internal fixation, locking plate group (39 cases) with locking plate fixation in the treatment of two groups. Statistical perioperative indicators (operation time, amount of bleeding, hospitalization time, fracture healing time, full weight-bearing time), tibial plateau malformation (TPA), within the dihedral angle (PA), HSS score and recovery results.] operation time of the two groups, the amount of bleeding during operation, hospitalization time, fracture healing time, full weight bearing time comparison, the difference was not statistically significant (P0.05). The patients were followed up for 1~2 years, two groups immediately after surgery, 2 months after surgery, 6 months after operation TPA, PA comparison, the difference was not statistically significant (P0. 05). Two groups were compared after 1 years when the HSS score, the difference was not statistically significant (P0.05). The two group comparison of the recovery of joint function, the difference was not statistically significant (P0.05). Conclusion: the anatomical plate and locking plate fixation of complex tibial plateau fracture clinical effect, clinical reasonable choice of surgical procedures should be based on specific the situation of patients.
