【摘要】：目的探討經(jīng)皮肝膽囊穿刺置管引流術(shù)(PTGCD)對(duì)老年急性膽囊炎患者炎癥指標(biāo)與胃腸功能的影響。方法回顧性分析該院收治的52例老年急性膽囊炎患者,根據(jù)治療方法分為PTGCD組和對(duì)照組,對(duì)照組22例采取膽囊造瘺術(shù)進(jìn)行治療,PTGCD組30例在超聲輔助下行PTGCD術(shù),比較兩組患者術(shù)前1 d和術(shù)后3 d血清白細(xì)胞介素(IL)-2、IL-4、IL-6水平,并記錄患者術(shù)后肛門排氣時(shí)間、排便時(shí)間和進(jìn)食固體食物的時(shí)間。結(jié)果 PTGCD組患者均經(jīng)一次置管成功,術(shù)后無明顯不良并發(fā)癥。與術(shù)前比較,兩組患者術(shù)后血清IL-4、IL-6水平均有所上升,但PTGCD組上升的幅度較小。IL-2水平均有所降低(均P0.05)。PTGCD組術(shù)后平均肛門排氣時(shí)間、排便時(shí)間和進(jìn)食固體食物的時(shí)間均顯著低于對(duì)照組(P0.05)。結(jié)論 PTGCD治療結(jié)石性膽囊炎療效安全可靠,可有效降低炎癥因子水平,促進(jìn)腸胃功能恢復(fù)。
[Abstract]:Objective to investigate the effect of percutaneous transhepatic cholecystectomy (PTGCD) on inflammatory indexes and gastrointestinal function in elderly patients with acute cholecystitis. Methods 52 elderly patients with acute cholecystitis treated in our hospital were retrospectively analyzed. According to the treatment methods, the PTGCD group and the control group were divided into two groups. The control group (n = 22) received cholecystostomy and 30 patients in the control group (n = 30) were treated with ultrasound assisted PTGCD. The serum levels of IL-2IL-4 and IL-6 were compared between the two groups on the 1st day before operation and 3 days after operation. The time of anal exhaust, defecation and eating solid food were recorded after operation. Results the patients in PTGCD group were successfully placed tube once, and there were no obvious complications after operation. The serum levels of IL-4 and IL-6 increased in both groups, but the amplitude of increase in PTGCD group was smaller. The level of IL-2 was decreased in both groups (P0.05. PTGCD group), and the mean anal exhaust time was lower in the PTGCD group than that in the P0.05. PTGCD group. The time of defecation and the time of eating solid food were significantly lower than that of control group (P0.050.Conclusion PTGCD is safe and reliable in the treatment of calculous cholecystitis, it can effectively reduce the level of inflammatory factors and promote the recovery of gastrointestinal function.
