骨錨技術(shù)修復(fù)深層尺橈韌帶對遠側(cè)尺橈關(guān)節(jié)旋轉(zhuǎn)穩(wěn)定性影響的生物力學(xué)研究

【摘要】:目的通過生物力學(xué)測試探討骨錨技術(shù)修復(fù)深層尺橈韌帶于尺骨凹對于恢復(fù)遠側(cè)尺橈關(guān)節(jié)旋轉(zhuǎn)穩(wěn)定性的重要性,為選擇手術(shù)方法提供生物力學(xué)理論依據(jù)。方法取9具新鮮成年尸體上肢標本,按照完整標本(正常組)、深層尺橈韌帶損傷(損傷組)、螺釘固定并肌腱縫線縫合修復(fù)深層尺橈韌帶(修復(fù)組)順序制備模型后,采用AG-IS系列MS型生物力學(xué)測試系統(tǒng),分別于腕關(guān)節(jié)背伸45°、中立位及屈曲45°進行內(nèi)、外旋生物力學(xué)測試,測量其扭矩。統(tǒng)計分析比較各組內(nèi)、外旋扭矩差異。結(jié)果腕關(guān)節(jié)背伸45°、中立位及屈曲45°時,正常組內(nèi)旋扭矩分別為(0.83±0.33)、(0.86±0.34)、(0.36±0.30)N·m,外旋扭矩分別為(0.86±0.38)、(0.44±0.22)、(0.25±0.21)N·m;損傷組內(nèi)旋扭矩分別為(0.18±0.17)、(0.22±0.17)、(0.16±0.15)N·m,外旋扭矩分別為(0.27±0.26)、(0.13±0.17)、(0.04±0.04)N·m;修復(fù)組內(nèi)旋扭矩分別為(0.79±0.34)、(0.73±0.33)、(0.41±0.23)N·m,外旋扭矩分別為(0.80±0.39)、(0.41±0.22)、(0.41±0.40)N·m。腕關(guān)節(jié)背伸45°、中立位及屈曲45°時,損傷組內(nèi)、外旋扭矩顯著低于正常組和修復(fù)組,差異均有統(tǒng)計學(xué)意義(P0.05);3種位置下修復(fù)組內(nèi)、外旋扭矩與正常組比較差異均無統(tǒng)計學(xué)意義(P0.05)。結(jié)論深層尺橈韌帶是維持遠側(cè)尺橈關(guān)節(jié)旋轉(zhuǎn)穩(wěn)定性的重要結(jié)構(gòu),骨錨技術(shù)修復(fù)深層尺橈韌帶于尺骨凹止點,對于恢復(fù)遠側(cè)尺橈關(guān)節(jié)旋轉(zhuǎn)穩(wěn)定性有著重要意義。
[Abstract]:Objective to explore the importance of repairing deep ulnar radial ligament in ulnar fovea by biomechanical test for the restoration of rotational stability of distal ulnar radial joint. To provide biomechanical theoretical basis for the selection of surgical methods 9 fresh adult cadaveric upper limb specimens were collected. The models were made according to the order of intact specimens (normal group, deep ulnar radial ligament injury group, screw fixation and tendon suture repair deep ulnar radial ligament repair group). AG-IS series MS biomechanical test system was used. The torque of wrist was measured at 45 擄dorsal extension, 45 擄neutral position and 45 擄flexion. The torque of the wrist was measured by external rotation biomechanics, and the difference of internal and external rotation torque was statistically analyzed and compared. Results the wrist dorsal extension was 45 擄, neutral position and flexion 45 擄. In the normal group, the internal rotation torque was 0.83 鹵0.33, the external rotation torque was 0.36 鹵0.30, and the external rotation torque was 0.86 鹵0.38, 0.44 鹵0.22, 0.25 鹵0.21, 0.18 鹵0.17, 0.16 鹵0.15, 0.27 鹵0.26, 0.13 鹵0.17, 0.04 鹵0.04N 路m, 0.79 鹵0.34 鹵0.33 鹵0.23 鹵0.23 N 路m, 0.80 鹵0.390.41 鹵0.23 N 路m, 0.80 鹵0.39 鹵0.2241 鹵0.22 鹵0.41 鹵0.41 鹵0.4045 鹵0.4045, respectively. The external rotation torque was significantly lower than that in the normal group and the repair group, and the difference was statistically significant under the three positions. Conclusion the deep ulnar radial ligament is an important structure to maintain the rotational stability of the distal ulnar radial joint, and bone anchor technique is used to repair the deep ulnar radial ligament at the end of the ulnar fossa. It is of great significance to restore the rotational stability of distal ulnar and radial joint.
