【摘要】：目的探討鎖定加壓鋼板(LCP)外固定對(duì)股骨遠(yuǎn)端骨折術(shù)后感染的效果。方法選擇2012年3月-2015年4月醫(yī)院骨科利用LCP外固定技術(shù)結(jié)合置管沖洗負(fù)壓引流技術(shù),對(duì)股骨遠(yuǎn)端骨折內(nèi)固定手術(shù)后發(fā)生感染的12例患者的臨床資料進(jìn)行分析。結(jié)果 12例骨折患者中,平均(44.00±10.50)歲;首次手術(shù)后到發(fā)生感染行LCP外固定的平均間隔時(shí)間為(16.17±2.73)d;所有患者均完成整個(gè)隨訪過程,隨訪時(shí)間平均為(15.50±1.51)月;患者骨折平均愈合時(shí)間為(19.17±2.25)周,全部病例均為骨性愈合,感染也全部得到有效控制;患者在手術(shù)后6個(gè)月膝關(guān)節(jié)屈曲角度平均為(110.17±10.66)°,伸直角度平均為(2.83±2.29)°;手術(shù)后1年的膝關(guān)節(jié)特種外科醫(yī)院評(píng)分(HSS)平均(90.17±5.56)分。結(jié)論利用LCP外固定技術(shù)結(jié)合置管沖洗負(fù)壓引流技術(shù)對(duì)股骨遠(yuǎn)端骨折內(nèi)固定手術(shù)后的感染進(jìn)行控制和治療,具有創(chuàng)傷比較小,在Ⅱ期并不需要進(jìn)行內(nèi)固定更換,手術(shù)后并發(fā)癥少而且節(jié)省住院費(fèi)用等優(yōu)點(diǎn),作為一種對(duì)感染性骨折的治療方法,有效性和安全性較高。
[Abstract]:Objective to investigate the LCPs of locking compression plate. Methods from March 2012 to April 2015, the orthopaedic department of the hospital used LCP external fixation technique combined with the drainage technique of irrigation and negative pressure. The clinical data of 12 patients with infection after internal fixation of distal femoral fractures were analyzed. Results the mean age of 12 patients with fractures was 44.00 鹵10.50 years old. The average interval between the first operation and the infection with LCP was 16.17 鹵2.73 days. All the patients completed the whole follow-up process, the average follow-up time was 15.50 鹵1.51 months. The average healing time of fracture was 19.17 鹵2.25 weeks. All cases were bony union and infection was effectively controlled. The average flexion angle of knee joint was 110.17 鹵10.66 擄and the average extension angle was 2.83 鹵2.29 擄6 months after operation. One year after surgery, the average score of HSS in special surgery hospital of knee joint was 90.17 鹵5.56). Conclusion the infection of distal femoral fracture after internal fixation was controlled and treated by LCP external fixation combined with catheter irrigation and negative pressure drainage. It has the advantages of small trauma, no need for internal fixation replacement in stage 鈪，
