【摘要】：目的評估機器人輔助腹腔鏡肝切除術(shù)的臨床療效。方法回顧性分析復(fù)旦大學(xué)附屬中山醫(yī)院2010年3月至2016年12月行機器人輔助腹腔鏡肝切除術(shù)的142例病人的臨床資料。結(jié)果原發(fā)性肝癌62例,結(jié)直腸癌肝轉(zhuǎn)移55例,肝門部膽管癌1例,肝良性病變23例,活體肝移植供體1例�！�3個肝段的大范圍肝切除占36.6%,3個肝段的部分肝切除占63.4%。無中轉(zhuǎn)開腹手術(shù)。出血量(119.0±121.8)mL,輸血率1.4%。R0切除率100%,圍手術(shù)期并發(fā)癥發(fā)生率(GradeⅢ)2.8%,病死率為0。術(shù)后住院(5.5±2.2)d。原發(fā)性肝癌病人術(shù)后1、3、5年總體存活率為97.8%、90.8%、76.2%,結(jié)直腸癌肝轉(zhuǎn)移病人術(shù)后1、2年總體存活率為97.8%、97.8%。結(jié)論對于適宜病例,機器人輔助腹腔鏡肝切除治療肝腫瘤安全可靠,近期及遠期療效良好。
[Abstract]:Objective to evaluate the clinical effect of robot-assisted laparoscopic hepatectomy. Methods retrospective analysis of the clinical efficacy of robot-assisted laparoscopic hepatectomy in Zhongshan Hospital affiliated to Fudan University from March 2010 to December 2016. Results there were 62 cases of primary liver cancer. There were 55 cases of liver metastasis, 1 case of hilar cholangiocarcinoma, 23 cases of benign hepatic lesion and 1 case of living donor liver transplantation. 36.6% cases were resected by large scale liver resection with more than 3 segments of liver. Partial hepatectomy accounted for 63.4% of the three segments. There was no conversion to open surgery. The volume of blood loss was 119.0 鹵121.8 mL.The blood transfusion rate was 1.4%. The resection rate of R0 was 100%. The incidence of perioperative complications was Grade 鈪，
