文章摘要:目的:探討我國醫(yī)院不同病區(qū)的護士特征分布以及護理工作環(huán)境情況。方法:該研究為描述性研究,采用一般資料調(diào)查表和護理工作環(huán)境量表(PES-NWI量表)對181家醫(yī)院共9698名護士進行調(diào)查,在病區(qū)層面上描述內(nèi)科、外科和ICU病區(qū)護士特征的分布情況,比較不同病區(qū)護理工作環(huán)境的差別。結(jié)果:ICU的護士較內(nèi)科、外科護士年輕,工作年限短,男護士及合同制護士比例大、學(xué)歷高。各類病區(qū)得分最高的均是“醫(yī)護合作關(guān)系”維度,得分最低的是“充足的人力和物力”維度,其次是“護士參與醫(yī)院事務(wù)”維度。ICU護士的PES-NWI量表總分以及各維度均分較內(nèi)科、外科均低,外科“人力和物力的充足l生”維度得分高于內(nèi)科(P〈O.001)。結(jié)論:ICU的護理工作環(huán)境較內(nèi)科和外科差;外科在“人力物力充足性”方面好于內(nèi)科。
Abstr:Objective: To describe and compare nurses characteristics and nursing work environment in medical wards, surgical wards and intensive care units in hospitals in mainland China. Methods: A descriptive design was used in this study. Totally 9698 clinical first-line nurses from 181 hospitals in mainland China were recruited and investigated by demographic questionnaire and Practice Environment Scale of the Nursing Work Index(PES-NWI). Results: Nurses in intensive care units were relatively younger and had less working years than those in medical and surgical wards. The percentages of male nurses, contract nurses and nurses with higher educational levels in intensive care units were higher than those in medical and surgical units. In all the three wards, the scores of "Collegial Nurse-Physician Relations" subscale of PES-NWI were the highest, while the scores of the "Staffing and Resource Adequacy" and "Nurse Participation in Hospital Affairs" subseales were the lowest. The total scores and means of PES-NWI and its subscales in intensive care units were lower than those in medical and surgical wards (P〈0.001). The scores of"Staffing and Resource Adequacy" subscale of PES-NWI in surgical wards were higher than those in medical wards (P〈0.001). Conclusions: The nursing work environment in intensive care units was worse than that in medical and surgical units. In terms of "Staffing and Resource Adequacy", surgical wards were better than medical wards.
